Betamethasone valerate 0.1%, neomycin sulphate 0.5%
Indications/Uses
Treatment of conditions where secondary bacterial infection is present, suspected, or likely to occur: Eczema in adults & childn ≥2 yr, including atopic & discoid eczemas; prurigo nodularis; psoriasis (excluding widespread plaque psoriasis); neurodermatoses including lichen planus; seborrhoeic dermatitis; contact sensitivity reactions. Management of insect bite reactions, prickly heat, anal & genital intertrigo.
Dosage/Direction for Use
Adult & childn ≥2 yr Apply bd-tds until improvement occurs. It may then be applied once daily or less often to maintain improvement. Do not continue for >7 days w/o medical supervision. Max 5 days when applied on face & used in childn.
Contraindications
Rosacea; acne vulgaris; perioral dermatitis; perianal & genital pruritus; pruritus w/o inflammation. Viral skin infections; primary infected skin lesions caused by fungal or bacterial infection; primary or secondary yeast infections; secondary infections due to Pseudomonas or Proteus spp. Otitis externa w/ tympanic membrane perforation. Long-term use at high doses or over large skin surface. Childn <2 yr.
Special Precautions
Patients w/ history of local hypersensitivity. Increased risk of skin hypersensitivity at application site w/ prolonged or frequent use. Avoid long-term treatment, particularly in childn <12 yr, due to risk of reversible HPA axis suppression & occurrence of symptoms of Cushing's syndrome. Avoid applying to eyelids. Discontinue in case of secondary infection of inflammatory lesions. Cleanse skin before occlusive dressing is applied. Not recommended in pregnant & breast-feeding women. Betamethasone valerate: Abrupt treatment w/drawal may result in glucocorticosteroid insufficiency. Negative effects in psoriasis. The face may exhibit atrophic changes after treatment. Risk of visual disturbances eg, cataract, glaucoma or rare diseases eg, central serous chorioretinopathy. Skin infection may persist due to masking effect. Higher incidence of local hypersensitivity reactions & increased risk of local infection when used to treat dermatitis around chronic leg ulcers. Neomycin sulphate: Risk of mild to life-threatening pseudomembranous colitis; nephrotoxicity & irreversible ototoxicity. Patients w/ renal impairment.
Adverse Reactions
Drug Interactions
Betamethasone valerate: Inhibited metabolism w/ CYP3A4 inhibitors (eg, ritonavir, itraconazole). Neomycin sulphate: Increased & prolonged resp system depression w/ depolarizing muscle relaxants. Accumulated toxicity w/ systemic aminoglycosides.
