Tolbutamide
Indications/Uses
Uncomplicated, stable, mild or moderately severe, non-ketotic, maturity-onset type DM which cannot be controlled by diet alone.
Dosage/Direction for Use
Initially 3 g on the 1st day, then 2 g on the 2nd day & 1 g on the 3rd day or 1-1.5 g daily. Maintenance: 0.5-1.5 g daily but may range 0.25-3 g daily. Total daily dose may be taken in the morning or in divided doses.
Administration
Should be taken with food.
Contraindications
Juvenile or growth onset DM. Severe or unstable "brittle" diabetes. Diabetic complicated by ketosis & acidosis, diabetic coma, major surgery, severe infection or trauma. Severe hepatic, renal or thyroid function impairment. Pregnancy.
Special Precautions
Lactation. Elderly.
Adverse Reactions
Nausea, vomiting, epigastric pain, dizziness, weakness & paraesthesia; hypersensitivity reactions w/ fever, eosinophilia, jaundice, skin rashes & blood disorders including leucopenia, thrombocytopenia, aplastic anemia & agranulocytosis; facial flushing (intolerance to alcohol).
Drug Interactions
Dicoumarol, sulphaphenazole, chloramphenicol, oxyphenbutazone, phenylbutazone, sulphadiazine, methyldopa, alcohol (acute intake); sulphadimethoxine, rifampicin; chlorpromazine, phenytoin; salicylic acid, ethionamide, aminosalicylic acid.
