Indications/Uses
Type II DM.
Dosage/Direction for Use
1-8 mg daily. Initial dose & dose titration: 1 mg once daily. Daily dose may be increased at intervals of 1-2 wk & carried out stepwise as follows: 1 mg, 2 mg, 3 mg, 4 mg, 6 mg & in exceptional cases, 8 mg.
Administration
Should be taken with food.
Contraindications
Type I DM, diabetic ketoacidosis, diabetic pre-coma or coma; severe impairment of renal or hepatic function. Pregnancy & lactation.
Special Precautions
Regular monitoring of glucose levels in blood & urine. Milder or absent symptoms of hypoglycemia in patients w/ autonomic neuropathy or taking β-blockers, clonidine, reserpine, guanethidine or other sympatholytics. Temporary changeover to insulin in exceptional stress situations (eg trauma, surgery, infections w/ fever).
Adverse Reactions
Hypoglycemia; temporary visual impairment; GI disturbances; skin rash; rarely thrombopenia, leucopenia, or hemolytic anemia.
Drug Interactions
Potentiation of blood sugar-lowering effect by insulin & other oral antidiabetics, ACE inhibitors, allopurinol, anabolic steroids & male sex hormones, cyclophosphamide, fibrates, guanethidine, MAOIs, miconazole, salicylates, sulphonamides & tetracyclines. Weakening of the blood sugar-lowering effect by acetazolamide, barbiturates, corticosteroids, diazoxide, diuretics, epinephrine & other sympathomimetic agents, glucagons, laxatives, oestrogens & progestogens, phenothiazines, phenytoin, rifampicin & thyroid hormones.
