Lignocaine HCl
Indications/Uses
Lubricant for catheters, endoscopes or other medical instruments for adults, adolescents & childn (2-12 yr). Local anaesth.
Dosage/Direction for Use
Individualized dosing. Application in the urethra before introducing a catheter, endoscope or other medical instruments Adult male 12.5 g. Childn 2-12 yr & high-risk patient Max dose: 2.9 mg/kg. Use for general anaesth & tracheal intubation Adult & adolescent >12 yr Distribute approx 5 g across the bottom 3rd of the tube immediately before administration. Max dose: 16 g. Childn 2-12 yr & high-risk patient Max dose: 2.9 mg/kg.
Contraindications
Hypersensitivity to lignocaine HCl & other local anaesth (amide type). Bulbocavernous reflux. Severe cardiac insufficiency, strongly decelerated heartbeat, AV block, shock due to cardiac failure or reduced blood vol. Methemoglobinemia. Childn <2 yr.
Special Precautions
Cardiac or resp impairment. Repeated or high dose application may result in severe side effects. Wounded or injured mucous membrane or an ulcer or inflammation in the area of intended application. Weak or acutely sick. Patient prone to seizures. Myasthenia gravis. Concomitant use w/ medicines for cardiac arrhythmias (eg, amiodarone). Porphyria. May impair swallowing when applied in oral cavity or pharynx. Tongue/mouth numbness may result in bite injuries. Increased absorption through mucous membrane & consequently severe side effects if a larger quantity was used than prescribed. Do not get in contact w/ eyes. Lubricant w/o lignocaine is preferred for general anaesth. Influence on the ability to drive & use machines cannot be excluded in the event of increased personal sensitivity. Severe liver or kidney dysfunction. Not recommended for repeated use during pregnancy & lactation; 12-hr interval between use & subsequent breastfeeding is recommended. Elderly.
Drug Interactions
Potential mutual intensified effects w/ medicines containing lignocaine or other local anaesth (amide type). Possible increased effect on the heart w/ antiarrhythmics [eg, mexiletine, tocainide, β-blockers (eg, propranolol) or Ca-channel blockers (eg, diltiazem & verapamil)]. Increased cardiac effects of class III antiarrhythmics (eg, amiodarone). Increased risk of side effects w/ H2-blockers (eg, cimetidine). Potential mutual intensified effects w/ medicines that may induce methemoglobinemia.
