Imipramine HCl
Indications/Uses
Neurotic or psychotic depressive states of diverse psychopathology & varying degree including organic-diseases associated depression, endogenous & reactive depression, involutional melancholia, senile depressions, depressive phase of manic-depressive psychosis & certain cases of depression for which a psychiatric basis has been established (ie, schizophrenia, alcoholism, mental deficiency). In place of electroconvulsive therapy (ECT), or supplement to ECT to prevent recurrent attacks. Persistent functional nocturnal enuresis when organic causes have been excluded in childn.
Dosage/Direction for Use
Depression Adult 25 mg tds, increase up to 150 mg if necessary Max: 200 mg daily. More severe, hospitalised case: 300 mg daily. Elderly & adolescent 30-40 mg daily, initially increased by 10 mg daily. Max: 100 mg (elderly). Maintenance: May be administered as a single dose before bedtime. Persistent functional enuresis irresponsive to other forms of management Childn ≥5 yr 10-25 mg 1 hr before bedtime for 2-4 wk, increase up to 50 mg if there is no response (childn 12-15 yr).
Contraindications
Hypersensitivity. Concomitant use of MAOIs.
Special Precautions
Patients w/ coronary thrombosis, angina pectoris, CHF, cardiac rate or rhythm or conduction disorders, prostatic disorders w/ potential urinary retention & glaucoma; hyperactive or agitated patients & seriously depressed epileptics or ambulatory patients w/ suicidal tendencies. Possible suicide in depressed patients during treatment in large quantities. Not to be used during pregnancy (1st trimester). Avoid during lactation. Elderly, childn & those w/ circulatory liability or CV disease.
Adverse Reactions
Hypotension, HTN, tachycardia, palpitation, MI, arrhythmias, heart block, stroke, falls; confusional states (especially in the elderly) w/ hallucinations, disorientation, delusions, anxiety, restlessness, agitation, insomnia, nightmares, hypomania, exacerbation of psychosis; numbness, tingling, paresthesias of extremities, incoordination, ataxia, tremors, peripheral neuropathy, extrapyramidal symptoms, seizures, EEG patterns alterations, tinnitus; dry mouth, constipation, paralytic ileus, urinary retention, delayed micturition, urinary tract dilation; skin rash, petechiae, urticaria, itching, photosensitization, edema (general or of face & tongue), drug fever, cross-sensitivity w/ desipramine; bone marrow depression including agranulocytosis, eosinophilia, purpura, thrombocytopenia; nausea & vomiting, anorexia, epigastric distress, diarrhoea, peculiar taste, stomatitis, abdominal cramps, black tongue; gynecomastia (male), breast enlargement & galactorrhea (female), increased or decreased libido, impotence, testicular swelling, blood sugar levels elevation or depression; jaundice (simulating obstructive), urinary frequency, drowsiness, dizziness, weakness & fatigue, headache, parotid swelling, alopecia.
Drug Interactions
Possible serious or fatal AR potentiation w/ MAOIs. May block the pharmacologic effects of guanethidine or similar agents.
