Indications/Uses
Chronic management of hyperuricemia in adults w/ gout in conditions where urate deposition has already occurred (including history or presence of tophus &/or gouty arthritis).
Dosage/Direction for Use
Initially 40 mg once daily. Patient who does not achieve serum uric acid <6 mg/dL after 2 wk 80 mg once daily. Severe renal impairment 40 mg once daily. Gout flare prophylaxis Flare prophylaxis w/ NSAID or colchicine is recommended upon initiation for up to 6 mth.
Administration
May be taken with or without food: May be taken w/o regard to antacid use.
Contraindications
Patients being treated w/ azathioprine or mercaptopurine.
Special Precautions
Discontinue use if serious skin reactions are suspected. Not recommended in asymptomatic hyperuricemia; patients whom rate of urate formation is greatly increased eg, malignant disease & its treatment, Lesch-Nyhan syndrome. Increased risk of CV death in gout patients w/ established CV disease. Concurrent prophylaxis w/ NSAID or colchicine is recommended when treatment is initiated. Obtain liver test panel (serum ALT, AST, alkaline phosphatase & total bilirubin) as baseline prior to initiation. Promptly monitor liver tests if symptoms that may indicate liver injury are reported. Exercise caution before driving, using machinery or participating in dangerous activities. Pregnancy & lactation. Childn <18 yr.
Adverse Reactions
Liver function abnormalities; dizziness, diarrhea.
Drug Interactions
May increase plasma conc of xanthine oxidase substrate drugs eg, azathioprine, mercaptopurine & theophylline.
