Contents
Per 2 g/500 mg Piperacillin Na 2 g, tazobactam Na 250 mg. Per 4 g/500 mg Piperacillin Na 4 g, tazobactam Na 500 mg
Indications/Uses
Treatment of systemic & local bacterial infections caused by susceptible organisms: Urinary tract, lower resp tract, intra-abdominal, skin & skin structure infections; bone & joint infections; gynecological infections; bacterial infections in neutropenic patients; bacterial septicemia; polymicrobic infections. Childn <12 yr: Neutropenia of intra-abdominal infection.
Dosage/Direction for Use
IV over 30 min Renal insufficiency Dosage recommendation: CrCl >40-90 mL/min 12 g/1.5 g/day in divided doses of 3.375 g 6 hrly, 20-40 mL/min 8 g/1 g/day in divided doses of 2.25 g 6 hrly, <20 mL/min 6 g/0.75 g/day in divided doses of 2.25 g 8 hrly. Duration of therapy: 7-10 days (usual), 7-14 days (nosocomial pneumonia). Hemodialysis Max dose: 2.25 g 8 hrly. Additional 0.75 g-dose after each dialysis.
Contraindications
History of allergic reactions to penicillins, cephalosporins or β-lactamase inhibitors.
Special Precautions
Renal insufficiency. Patients requiring restricted salt intake; w/ low K reserves & receiving cytotoxic therapy or diuretics. Pregnancy & lactation. Childn.
Adverse Reactions
Hypotension, ileus, syncope; rigors, back pain, malaise; tachycardia, bradycardia, arrhythmia, cardiac or circulatory failure, MI; tremor, convulsions, vertigo, melena, flatulence, hemorrhage, gastritis, hiccups, ulcerative stomatitis; tinnitus; anaphylaxis; symptomatic hypoglycemia, thirst; myalgia, arthralgia; mesenteric embolism, purpura, epistaxis, pulmonary embolism; confusion, hallucination, depression; leucorrhea, vaginitis; pharyngitis, pulmonary edema, bronchospasm, coughing; genital pruritis, diaphoresis; taste perversion; urinary retention, dysuria, oliguria, hematuria, incontinence; photophobia, flushing, hepatitis, cholestatic jaundice; hemolytic anemia.
Drug Interactions
Aminoglycosides, probenecid, heparin, other oral anticoagulant, vecuronium.
