Indications/Uses
Short-term treatment of severe anxiety disorders; hypnotic in the short-term management of insomnia; sedative & premedicant; anticonvulsant in the control of muscle spasms; management of withdrawal symptoms.
Dosage/Direction for Use
Severe anxiety 2 mg tid. Max: 30 mg daily. Childn >6 mth receiving up to 10 mg daily Dose range: 4-40 mg daily in divided doses. Short-term management of insomnia associated w/ anxiety 5-15 mg at bedtime, up to 30 mg may be given. Childn & adolescent 12-18 yr 1-5 mg at bedtime. Premed 5-15 mg. Childn 5-12 yr 5 mg, 1-5 yr 2.5 mg, 1 mth-1 yr 250 mcg/kg. Sedation during minor surgical & medical procedures Childn & adolescent 12-18 yr 100-200 mcg/kg (max: 20 mg), 1 mth-12 yr 200-300 mcg/kg (max: 5 mg). Seizures (as adjunct in some types of epilepsy) 2-60 mg daily in divided doses. Muscle spasm Adult 2-15 mg daily in divided doses. Dose may be increased up to 60 mg daily in severe spastic disorder eg cerebral palsy. Childn & adolescent 12-18 yr 10 mg bid (max: 40 mg daily), 5-12 yr 5 mg bid, 1-5 yr 2.5 mg bid, 1-12 mth 250 mcg/kg bid. Symptoms of alcohol w/drawal syndrome 5-20 mg repeated, if required, after 2-4 hr. Alternative: 10 mg tid or qid on the 1st day, reducing to 5 mg tid as required.
Administration
May be taken with or without food.
Contraindications
Preexisting CNS depression or coma, resp depression, acute pulmonary insufficiency, myasthenia gravis or sleep apnea. Chronic psychosis, phobic or obsessional states. As monotherapy in depression or anxiety associated w/ depression. Glaucoma. Severe hepatic impairment. Pregnancy & lactation.
Special Precautions
Chronic pulmonary insufficiency. Patients w/ personality disorders, muscle weakness; hepatic or renal impairment. Dependence particularly in patients w/ history of alcohol or drug abuse. May impair ability to drive or operate machinery. Elderly or debilitated patients.
Adverse Reactions
Drowsiness, sedation, muscle weakness, ataxia; vertigo, headache, confusion, depression, slurred speech or dysarthria, changes in libido & salivation, tremor, visual & GI disturbances, urinary retention or incontinence, amnesia.
Drug Interactions
CNS depressants eg alcohol, antidepressants, sedative antihistamines, antipsychotics, general anesth, other hypnotics or sedatives, opioid analgesics; cisapride. Drugs that interfere metabolism of benzodiazepines.
