Indications/Uses
Treatment of resp tract, skin & soft tissue infections, leprosy, infections due to Mycobacterium avium, protozoal infection & for the eradication of H. pylori associated w/ peptic ulcer.
Dosage/Direction for Use
Tab Adult 250 mg bid, may be increased to 500 mg bid in severe infection for 7-14 days. Mycobacterium avium complex Prophylaxis/Treatment: Adult 500 mg bid. Childn 7.5 mg/kg bid up to 500 mg bid. Leprosy 500 mg daily as part of an alternative multidrug therapy regimen. Eradication of H. pylori associated w/ peptic ulcer 500 mg bid given w/ another antibacterial & either a proton-pump inhibitor or an H2-receptor antagonist for 7-14 days. Susp Childn 10-12 yr (30-40 kg) 10 mL (2 tsp); 7-9 yr (20-29 kg) 7.5 mL (1½ tsp); 3-6 yr (12-19 kg) 5 mL (1 tsp); 1-2 yr (8-11 kg) 2.5 mL (½ tsp); <8 kg 7.5 mg/kg daily. Duration of treatment: 5-10 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to macrolide antibiotics. Concomitant use w/ astemizole, cisapride & terfenadine.
Special Precautions
Consider diagnosis of pseudomembranous colitis in patients who develop diarrhea. Impaired renal & hepatic function. Pregnancy & lactation.
Adverse Reactions
GI disturbances eg, nausea, dyspepsia, abdominal pain, vomiting & diarrhea.
Drug Interactions
Increased toxicity of rifabutin. Delirium may occur w/ fluoxetine. Metabolism inhibited w/ ritonavir. Decreased plasma conc & high incidence of skin rashes w/ efavirenz. Altered pharmacokinetic parameters w/ cimetidine. Increased conc w/ omeprazole. Enhanced activity w/ antimycobacterials (eg, ethambutol, INH, pyrazinamide & rifampicin). Increased serum conc of theophylline & digoxin. Increased plasma conc of carbamazepine & terfenadine. Increased mean steady-state Cmin & AUC w/ fluconazole. Potentiate effects of oral anticoagulants. Increased toxicity of colchicines. Torsades de pointes may occur w/ quinidine or disopyramide. Acute toxicity w/ ergotamine or dihydroergotamine. CNS effects w/ triazolam, alprazolam & other benzodiazepines. Increased conc of lovastatin & simvastatin. Increase AUC of sildenafil. Cyclosporine, tacrolimus, alfentanil, disopyramide, quinidine, methylprednisolone, cilostazol, bromocriptine, hexobarbital, phenytoin & valproate.
