Indications/Uses
Adult & childn >3 mth: Infections caused by susceptible organism including pneumonia; resp tract (including cystic fibrosis), UTI, intra-abdominal, intra- & post-partum, skin & soft tissue infections, & bacterial meningitis. Neutropenic patient w/ fever suspected to be due to bacterial infection.
Dosage/Direction for Use
Adult & adolescent Up to 2 g tid. Childn Nosocomial infection due to Pseudomonas aeruginosa or Acinetobacter spp Up to 40 mg/kg tid. Adult Pneumonia including community-acquired & nosocomial pneumonia 500 mg or 1 g every 8 hr. Bronchopulmonary infection in cystic fibrosis 2 g every 8 hr. Complicated UTI, intra-abdominal & skin & soft tissue; intra- & post-partum infections 500 mg or 1 g every 8 hr. Acute bacterial meningitis 2 g every 8 hr. Febrile neutropenic patient 1 g every 8 hr. Childn >50 kg Adult dose. Childn 3 mth-11 yr & up to 50 kg Pneumonia including community-acquired & nosocomial pneumonia 10 or 20 mg/kg every 8 hr. Bronchopulmonary infection in cystic fibrosis 40 mg/kg every 8 hr. Complicated UTI, intra-abdominal & skin & soft tissue; intra- & post-partum infections 10 mg or 20 mg/kg every 8 hr. Acute bacterial meningitis 40 mg/kg every 8 hr. Febrile neutropenic patient 20 mg/kg every 8 hr. Renal impairment Dose based on unit doses of 500 mg, 1 g, 2 g every 8 hr: CrCl 26-50 mL/min 1 u every 12 hr; 10-25 mL/min ½ u every 12 hr; <10 mL/min ½ u every 24 hr. Administer over approx 15-30 min. Alternatively, doses up to 1 g may be given IV bolus inj over approx 5 min.
Contraindications
Hypersensitivity to meropenem, other carbapenem or β-lactam antibacterial agents.
Special Precautions
Hypersensitivity. Overgrowth of nonsusceptible organisms; pseudomembranous colitis. +ve direct or indirect Coombs' test. Patient w/ preexisting liver disorders. May affect ability to drive or operate machinery. Pregnancy & lactation. Not recommended in infant <3 mth.
Adverse Reactions
Thrombocythemia; nausea, vomiting, diarrhea; increased serum transaminases, bilirubin, alkaline phosphatase & lactic dehydrogenase; inflammation, thrombophlebitis, pain.
Drug Interactions
Renal excretion is inhibited w/ probenecid. May reduce serum valproic acid levels.
