Indications/Uses
Source of water & electrolytes. Priming soln in hemodialysis procedures.
Dosage/Direction for Use
Special Precautions
Patients w/ CHF, severe renal insufficiency, & in clinical states in which there exists edema w/ Na retention; diminished renal function. Stop the infusion immediately if signs or symptoms of hypersensitivity/infusion reactions develop. Risk of fluid &/or solute overload & electrolyte disturbances. Patients w/ or at risk for hypernatremia, hyperchloremia, metabolic acidosis, hypervolemia; medications that may increase the risk of Na & fluid retention eg, corticosteroids; conditions that may cause Na retention, fluid overload & edema (central & peripheral) eg, patients w/ primary hyperaldosteronism, secondary hyperaldosteronism associated w/ HTN, CHF, liver disease (including cirrhosis), renal disease (including renal artery stenosis, nephrosclerosis) or pre-eclampsia. Risk of hyponatremia. Risk of air embolism. Severe renal impairment. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Febrile response, venous thrombosis or phlebitis extending from site of inj, extravasation, & hypervolemia. Hypersensitivity/infusion reactions including hypotension, pyrexia, tremor, chills, urticaria, rash, pruritus; infusion site reactions eg, infusion site erythema, inj site streaking, burning sensation, infusion site urticaria. Hypernatremia, hyperchloremic metabolic acidosis, hyponatremia which may be symptomatic, hyponatremic encephalopathy.
Drug Interactions
Increased renal Na & lithium clearance. Decreased lithium levels. May increase the risk of Na & fluid retention w/ corticosteroids. Increased vasopressin effect, leading to reduced renal electrolyte free water excretion & may increase the risk of hyponatremia w/ drugs stimulating vasopressin release eg, chlorpropamide, clofibrate, carbamazepine, vincristine, SSRIs, 3,4-methylenedioxy-N-methamphetamine, ifosfamide, antipsychotics, opioids; drugs potentiating vasopressin action eg, chlorpropamide, NSAIDs, cyclophosphamide; vasopressin analogues eg, desmopressin, oxytocin, vasopressin, terlipressin. May increase the risk of hyponatremia w/ diuretics & antiepileptics (eg, oxcarbazepine).
