Donepezil HCl
Indications/Uses
Moderate to severe dementia of the Alzheimer's type.
Dosage/Direction for Use
Initially 5 mg once daily. May administer 10 mg daily when daily dose has been 5 mg for 4-6 wk. Patient established on 10 mg daily for at least 3 mth 23 mg once daily in the evening prior to retiring.
Administration
May be taken with or without food: Take at bedtime. Swallow whole, do not split/crush.
Contraindications
Hypersensitivity to donepezil HCl or piperidine derivatives.
Special Precautions
Exaggerated succinylcholine-type muscle relaxation during anesth. Vagotonic effects on SA or AV nodes; syncopal episodes. Nausea & vomiting. PUD & GI bleeding; history of ulcer disease or concomitant use w/ NSAIDs. Wt loss. Bladder outflow obstruction. May cause generalized convulsions. History of asthma or obstructive pulmonary disease. Lower wt individuals. Pregnancy & lactation. Childn.
Adverse Reactions
Diarrhea, muscle cramps, fatigue, nausea, vomiting, insomnia; headache. Common colds; anorexia; hallucinations, agitation, aggressive behavior; syncope, dizziness; abdominal disturbance; rash, pruritus; urinary incontinence; pain; accident.
Drug Interactions
Inhibited metabolism w/ ketoconazole & quinidine. Increased conc w/ ketoconazole. Reduced clearance w/ a known CYP2D6 inhibitor. Increased rate of elimination w/ CYP2D6 & CYP3A4 inducers (eg, phenytoin, carbamazepine, dexamethasone, rifampicin & phenobarb). May interfere activity of anticholinergic medications. Synergistic effect w/ succinylcholine, similar neuromuscular blocking agents or cholinergic agonists eg, bethanechol.
