Indications/Uses
Hypovolemic shock. Adjunct in hemodialysis for patient undergoing long-term dialysis or who is fluid-overloaded & cannot tolerate substantial vol of salt soln for therapy of shock or hypotension. Cardiopulmonary bypass procedures. May be used for adult resp distress syndrome; major injury or surgery resulting in increased albumin loss or inadequate synthesis; acute nephrosis not responding to cyclophosphamide or steroid therapy; acute liver failure or ascites. Childn: Neonatal hyperbilirubinemia in conjunction w/ exchange infusion.
Dosage/Direction for Use
IV Individualized dosage. Adult Initially 100 mL. Patient in shock w/ greatly reduced blood vol May be administered as rapidly as necessary, may be repeated in 15-30 min if initial dose is inadequate. Patient w/ slightly low or normal blood vol Rate of administration: 1 mL/min. Childn Individualized dosage. ¼ to ½ the adult dose or dosage may be calculated on the basis of 0.6-1 g/kg. Jaundiced infant suffering from hemolytic disease of the newborn 1 g/kg. Rate of administration: ¼ the adult rate.
Contraindications
Hypersensitivity. Severe anemia, cardiac failure w/ normal or increased intravascular vol.
Special Precautions
Potentially fatal hemolysis & acute renal failure w/ inappropriate use of sterile water for inj as diluent. Risk for transmission of viral diseases including Creutzfeldt-Jakob disease. Patient w/ low cardiac reserve. Rapid infusion may cause vascular overload w/ resultant pulmonary edema; monitor patients for signs of increased venous pressure. Carefully observe injured or post-op patient for rapid rise in BP following infusion to detect & treat severed blood vessels that may not have bled at a lower pressure. Additional fluids in patient w/ marked dehydration. Not to be infused through the same administration set w/ soln containing protein hydrolysates or alcohol. Pregnancy.
Adverse Reactions
Fever & chills, rash, nausea, vomiting, tachycardia & hypotension. Pulmonary edema due to vascular overload if administered rapidly.
