Pneumococcal 10-valent polysaccharide conjugate vaccine (adsorbed)
Indications/Uses
Active immunization for prevention of invasive disease, pneumonia & acute otitis media caused by Streptococcus pneumoniae serotypes 1, 5, 6A, 6B, 7F, 9V, 14, 19A, 19F & 23F in infants & toddlers from 6 wk-2 yr.
Dosage/Direction for Use
0.5 mL IM (1 dose). Unvaccinated childn 12-24 mth 2 doses of 0.5 mL w/ at least 2-mth interval between doses, 7-11 mth 2 primary doses of 0.5 mL w/ at least 1-mth interval between doses, followed by booster (3rd) dose in 2nd yr w/ at least 2-mth interval after last primary dose. Infant & toddler 3-dose primary series: 1st dose given as early as 6 wk or at 2 mth w/ 4-8-wk interval. 2nd dose given at 10th wk w/ 4-8 wk interval. 3rd dose given at 14th wk w/ 4-8-wk interval. Booster dose given at least 6 mth after last primary dose & may be given from 9 mth onwards, preferably between 12-15 mth.
Contraindications
Hypersensitivity to vaccine including diphtheria toxoid.
Special Precautions
Not to be inj intravascularly, intradermally, SC, IV. Anaphylactic reaction. Risk of sensitization w/ thiomersal. Not to be inj IM in patients w/ thrombocytopaenia, any coagulation disorder or those receiving anticoagulant therapy. Not to be used for active infection. May not protect all individuals from pneumococcal disease. Low protection against otitis media. Postpone vaccination in patients suffering from acute severe febrile illness. Consider resp monitoring for 48-72 hr in very premature infants (born ≤28 wk of gestation) & w/ history of resp immaturity. Avoid use during pregnancy. Lactation. Childn w/ congenital or acquired splenic dysfunction, HIV infection, malignancy, nephrotic syndrome; sickle cell disease, born premature. Not to be used in childn <6 wk.
Adverse Reactions
Pain, fever ≥37.5°C (axillary); irritability. Inj site erythema, swelling/induration; decreased appetite; drowsiness; rash.
Drug Interactions
Reduced immune response w/ paracetamol.
