Promethazine HCl
Indications/Uses
Symptomatic treatment of allergic conditions of the upper resp tract & skin including allergic rhinitis, urticaria & anaphylactic reactions to drugs & foreign proteins. Adjunct in pre-op sedation in surgery & obstet. Antiemetic. Short-term sedation & treatment of insomnia in adults; paed sedative.
Dosage/Direction for Use
Antihistamine in allergic conditions Adult including elderly & childn >10 yr Initially 10 mg bd. Max: 20 mg tds as required, childn 5-10 yr 10-25 mg as single dose or 5-10 mg bd. Max: 25 mg daily, 2-5 yr 5-15 mg as single dose or 5 mg bd. Max: 15 mg daily. Antiemetic Adult including elderly & childn >10 yr 25 mg, childn 5-10 yr 10 mg, 2-5 yr 5 mg. All doses are to be taken at night before travel & are repeated 6-8 hr as required. Short-term sedation Adult including elderly & childn >10 yr 25 or 50 mg, childn 5-10 yr 20 or 25 mg, 2-5 yr 15 or 20 mg. All doses are to be taken as single nighttime dose.
Administration
May be taken with or without food.
Contraindications
Patients in coma, suffering from CNS depression of any cause, MAOI therapy w/in 14 days of discontinuation. Childn <2 yr.
Special Precautions
Asthma, bronchitis or bronchiectasis; severe CAD, narrow-angle glaucoma, epilepsy, hepatic & renal insufficiency; bladder neck or pyloroduodenal obstruction; signs & symptoms of Reye's syndrome in childn & adolescents; long-term use for >7 days. May mask warning signs of ototoxicity & may delay early diagnosis of intestinal obstruction or raised ICP. Avoid strong sunlight exposure. May affect ability to drive or operate machinery. Pregnancy & lactation. Childn 2-6 yr.
Adverse Reactions
Drowsiness, dizziness, restlessness, headache, nightmares, tiredness & disorientation; occasionally, anticholinergic effects; paradoxical hyperexcitability in some childn; confusion in the elderly; urticaria, rash, pruritus, anorexia, gastric irritation, palpitations, hypotension, arrhythmias, extrapyramidal effects, muscle spasms, tic-like head & face movements. Photosensitivity.
Drug Interactions
Anticholinergics, TCAs, sedative or hypnotics; alcohol.
