Lignocaine HCl
Indications/Uses
Local anaesth, dental infiltration or nerve block, local infiltration, peripheral nerve & sympathetic block, transtracheal anaesth.
Dosage/Direction for Use
Infiltration anaesth 2% soln w/ adrenaline 1 in 100,000 to 1 in 500,000. Nerve block 1-2 mL of 2% soln. Dentistry 2% soln w/ adrenaline in 1 in 100,000 or noradrenaline 1 in 800,000.
Contraindications
Hypersensitivity to amide-type local anaesth. Myasthenia gravis, hypovolaemia, complete heart block.
Special Precautions
Epilepsy, impaired cardiac conduction. Anaesth in appendages w/ adrenaline-containing soln. Administration into inflamed or infected area w/ low tissue pH. Hepatic damage. Pregnancy in presence of foetal hypoxia or acidosis & lactation. Childn. Elderly or debilitated patients.
Adverse Reactions
Allergic reaction; hypotension, bradycardia, cardiac arrest, CNS effects including agitation, euphoria, resp depression, convulsion, yawning, restlessness, excitement, nervousness, dizziness, blurred vision, nausea, vomiting; muscle twitching followed by depression w/ drowsiness, resp failure, coma.
Drug Interactions
Antagonised effects of antimyasthenics on skeletal muscle. Additive depressant effect w/ CNS depressants. Released heavy metal ions from disinfectant soln causing severe local irritation, swelling, oedema. Excessive circulation restriction w/ methoxamine. Ischaemia leading to gangrene w/ other vasoconstrictors eg, adrenaline or phenylephrine. Increased risk of lignocaine toxicity (reduced metabolism) w/ β-adrenergic blocking agents. Inhibition of hepatic metabolism w/ cimetidine.
