Clarithromycin
Indications/Uses
Nasopharynx tract infections (eg, tonsillitis, pharyngitis), paranasal sinuses. Lower resp tract infections eg, bronchitis, bacterial & atypical pneumonia. Skin infections eg, impetigo, erysipelas, folliculitis, furunculosis, septic wounds.
Dosage/Direction for Use
Adult 250 mg every 12 hr. Severe infections May be increased up to 500 mg every 12 hr for up to 6-14 days. Renal impairment CrCl <30 mL/min Reduce dose by ½, not to exceed for 14 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to clarithromycin, erythromycin or macrolides. Not to be co-administered w/ ergot derivatives. Not recommended w/ astemizole. Concomitant use w/ cisapride, pimozide, terfenadine, domperidone. Pregnancy.
Special Precautions
Discontinue use if severe acute hypersensitivity reactions occur. Not to be used in patients w/ history of acute porphyria. Pseudomembranous colitis. Cross-resistance w/ other macrolides, lincomycin, clindamycin. May affect ability to drive & use machines. Not recommended in concomitant use w/ ranitidine bismuth citrate in patients w/ CrCl <25 mL/min. Moderate to severe renal failure; hepatic impairment. Not to be used during 1st trimester of pregnancy. Lactation. Paed <6 mth. Elderly.
Adverse Reactions
GI disturbances, headache, rash, stomatitis, glossitis, oral monilia, pseudomembranous colitis, transient CNS events.
Drug Interactions
Increased serum conc of theophylline, digoxin. Increased plasma conc of carbamazepine, terfenadine. Increased steady-state plasma conc of omeprazole. Decreased steady-state zidovudine conc. Potentiated effects of oral anticoagulants. Elevated conc of CYP3A-metabolized drugs. Increased serum levels of warfarin, ergot alkaloids, midazolam, lovastatin, phenytoin, ciclosporin. Torsade de pointes w/ quinidine/disopyramide. QT prolongation & cardiac arrhythmias w/ domperidone. QT prolongation & Torsade de pointes w/ astemizole. Concomitant use w/ alprazolam, cisapride, methylprednisolone, pimozide, rifabutin, simvastatin, tacrolimus, triazolam.
