Indications/Uses
Upper & lower resp tract, skin & soft tissue, bone & joint, gynecological, intra-abdominal & dental infections; septicemia & endocarditis; toxoplasmic encephalitis & Pneumocystis jiroveci pneumonia in patients w/ AIDS.
Dosage/Direction for Use
Cap Adult 600-1,800 mg daily in 2, 3 or 4 equal doses. Childn from 2 yr 8-25 mg/kg daily in 3 or 4 equal doses. β-hemolytic streptococcal infections Continue treatment for at least 10 days. Chlamydia trachomatis cervicitis 450 mg qds for 10-14 days. Toxoplasmic encephalitis in patient w/ AIDS 600-1,200 mg 6 hrly IV/PO for 2 wk followed by 300-600 mg PO 6 hrly. Duration: 8-10 wk. Pyrimethamine dose: 25-75 mg PO daily for 8-10 wk. Folinic acid dose: 10-20 mg daily. Pneumocystis jiroveci in patient w/ AIDS 600-900 mg 6 hrly or 900 mg 8 hrly IV or 300-450 mg PO 6 hrly for 21 days. Primaquine dose: 15-30 mg PO once daily for 21 days. Acute streptococcal tonsillitis/pharyngitis 300 mg bd for 10 days. IM/IV Adult Serious infection 2,400-2,700 mg daily in 2, 3 or 4 equal doses. Less complicated infections 1,200-1,800 mg daily in 3 or 4 equal doses. Max dose: 4,800 mg daily. Single IM doses of >600 mg are not recommended. Pelvic inflammatory disease 900 mg 8 hrly w/ gm -ve aerobic antibiotic, continue w/ 450-600 mg PO 6 hrly to complete 10-14 days of therapy. Childn from 2 yr 20-40 mg/kg daily in 3 or 4 equal doses.
Administration
May be taken with or without food: Swallow whole w/ a full glass of water & in an upright position.
Contraindications
Hypersensitivity to clindamycin or lincomycin. Newborns.
Special Precautions
Discontinue use if hypersensitivity or severe skin reactions occur. Not to be used in meningitis, nonbacterial infections eg, URTI. Pseudomembranous & severe colitis; Clostridium difficile-associated diarrhea; overgrowth of nonsusceptible organisms. Perform hepatic & renal function test in prolonged therapy. Pregnancy & lactation. Soln for inj: Not to be inj IV as undiluted bolus. Fatal gasping syndrome in premature infants. Avoid use in childn <2 yr. Premature & low birth wt infants.
Adverse Reactions
Pseudomembranous colitis; eosinophilia; thrombophlebitis; diarrhoea; maculopapular rash; abnormal LFT.
Drug Interactions
Enhanced action of other neuromuscular blocking agents. Reduced clearance w/ CYP3A4/5 inhibitors. Increased clearance w/ CYP3A4/5 inducers. Monitor for loss of effectiveness w/ strong CYP3A4 inducers eg, rifampicin.
