Isotretinoin
Indications/Uses
Treatment of cystic, conglobate & severe acne which has failed to respond to other therapies.
Dosage/Direction for Use
Initially 0.5 mg/kg daily for 4 wk. May be increased to 1 mg/kg daily. Patients who shows intolerance to initial dose: 0.1-0.2 mg/kg body wt daily.
Administration
Should be taken with food.
Contraindications
Hepatic or renal impairment. Hypervitaminosis A, hyperlipidaemia. Nonconglobate, mild cystic & other non-severe forms of acne. Concomitant use w/ tetracyclines. Pregnancy & lactation. Childn <17 yr.
Special Precautions
Hepatic & hematologic monitoring in patients w/ lipid disorders, obesity, alcohol abuse, DM. Do not donate blood w/in 1 mth after therapy. May impair ability to drive or operate machinery.
Adverse Reactions
Dry skin, nasal mucosa & conjunctiva. Visual disturbances, allergic vasculitis, arthralgia, hepatitis. Elevation of liver enzymes, serum triglycerides, cholesterol. Hematuria & proteinuria. Rarely, keratitis, acne fulminans, hearing deficiency, hair thinning, hirsutism, nausea, headache, benign intracranial HTN, suicidal ideation, hyperuricemia, inflammatory bowel disease, blood disorders, bronchospasm.
Drug Interactions
Vit A, tetracyclines, diphenylhydantoin. Keratolytics & exfoliants. Radiation therapy.
