Indications/Uses
Treatment of oesophageal reflux disease, duodenal & benign gastric ulcers including those complicating NSAID therapy. Relief of associated dyspeptic symptoms. Prophylaxis of acid aspiration. Zollinger-Ellison syndrome. In combination w/ antibiotics for eradication of H. pylori in PUD.
Dosage/Direction for Use
Oesophageal reflux disease including reflux oesophagitis 20 mg once daily, can also be administered at 40 mg once daily in patients w/ reflux oesophagitis refractory to other therapy. Duration of treatment: 4-8 wk. Acid reflux disease 10 mg once daily for long-term treatment, increasing to 20 mg if symptoms return. Duodenal & benign gastric ulcers 20 mg once daily, may be increased to 40 mg once daily for severe or recurrent cases. Duration of therapy: 8 wk. Prevention of relapse in patients w/ duodenal ulcer 10 mg once daily, increasing to 20 mg once daily if symptoms return. Patient w/ H. pylori infection, patient <60 yr; symptoms persist for ≥1 yr & smokers Initially 20 mg once daily, reducing to 10 mg once daily. H. pylori eradication in PUD 40 mg once daily or 20 mg bd in combination w/ amoxicillin 500 mg & metronidazole 400 mg tds for 1 wk or clarithromycin 250 mg & metronidazole 400 mg (or tinidazole 500 mg) bd for 1 wk or amoxicillin 1 g & clarithromycin 500 mg bd for 1 wk or amoxicillin 750 mg to 1 g bd for 2 wk. Prophylaxis of acid aspiration 40 mg on the evening before surgery followed by 40 mg 2-6 hr prior to surgery. Zollinger-Ellison syndrome Initially 60 mg once daily. Dose should be adjusted individually & treatment continued as long as clinically indicated. Childn >2 yr Severe ulcerating reflux oesophagitis 0.7-1.4 mg/kg daily. Max: 40 mg/day for 4-12 wk.
Administration
Should be taken with food: Take immediately before a meal.
Contraindications
Possibility of malignancy should be excluded before treatment when gastric ulcer is suspected. Treatment may alleviate symptoms & delay diagnosis.
Special Precautions
Treatment w/ acid-reducing drugs may lead to a slightly increased risk of GI infections eg, Salmonella & Campylobacter. Pregnancy & lactation.
Adverse Reactions
Skin rash, urticaria, pruritus; paraesthesia.
Drug Interactions
Ketoconazole, itraconazole, diazepam, phenytoin, warfarin, clarithromycin, digoxin.
