Indications/Uses
Infections in adults & childn >3 mth: Severe pneumonia including hospital & ventilator-associated pneumonia; broncho-pulmonary infections in cystic fibrosis; complicated UTI; complicated intra-abdominal infections; intra- & post-partum infections; complicated skin & soft tissue infections; acute bacterial meningitis. Treatment of bacteraemia in association w/, or is suspected to be associated w/, any of the infections previously listed. May be used in the management of neutropenic patients w/ fever that is suspected to be due to a bacterial infection.
Dosage/Direction for Use
IV infusion over approx 15-30 min. Alternatively, doses up to 1 g (in adult) or 20 mg/kg (in childn) can be given as IV bolus inj over approx 5 min. Adult & adolescent Severe pneumonia including hospital & ventilator-associated pneumonia; complicated UTI; complicated intra-abdominal infections; intra- & post-partum infections; complicated skin & soft tissue infections 500 mg or 1 g every 8 hr. Broncho-pulmonary infections in cystic fibrosis; acute bacterial meningitis 2 g every 8 hr. Management of febrile neutropenic patients 1 g every 8 hr. Childn weighing >50 kg Administer adult dose. Childn 3 mth-11 yr & weighing up to 50 kg Severe pneumonia including hospital & ventilator-associated pneumonia; complicated UTI; complicated intra-abdominal infections; complicated skin & soft tissue infections 10 or 20 mg/kg every 8 hr. Broncho-pulmonary infections in cystic fibrosis; acute bacterial meningitis 40 mg/kg every 8 hr. Management of febrile neutropenic patients 20 mg/kg every 8 hr. Renal impairment, CrCl 26-50 mL/min 1 unit dose every 12 hr, CrCl 10-25 mL/min ½ unit dose every 12 hr, CrCl <10 mL/min ½ unit dose every 24 hr.
Contraindications
Hypersensitivity to meropenem or to any other carbapenem antibacterial agent. Severe hypersensitivity to any other type of β-lactam antibacterial agent (eg, penicillins or cephalosporins).
Special Precautions
History of hypersensitivity to carbapenems, penicillins or other β-lactam antibiotics. Discontinue if severe allergic reaction occurs. Immediately w/draw & consider alternative treatment if signs & symptoms suggestive of severe cutaneous adverse reactions eg, SJS, TEN, DRESS, erythema multiforme & acute generalised exanthematous pustulosis appear. Reports of antibiotic-associated colitis & pseudomembranous colitis; seizures. Risk of hepatic toxicity. Closely monitor hepatic function during treatment. May develop a +ve direct or indirect Coombs test. Not recommended w/ valproic acid/Na valproate/valpromide. Patients on a controlled Na diet. Patient w/ renal impairment. May affect ability to drive & use machines. Avoid during pregnancy. Should not be used in breast-feeding women. Childn <3 mth.
Adverse Reactions
Thrombocythaemia; headache; diarrhoea, abdominal pain, vomiting, nausea; increased transaminases, blood alkaline phosphatase, blood LDH; rash, pruritus; inflammation, pain.
Drug Interactions
Increased elimination t½ & plasma conc w/ probenecid. Decreased blood levels of valproic acid/Na valproate/valpromide. Increased effects of oral anticoagulants (eg, warfarin).
