Contents
Per L Purified soybean oil 200 g, organic phosphate 15 mmol. Energy: 2000 kcal
Indications/Uses
Supply of energy & essential fatty acids in IV nutrition. Patient w/ essential fatty acid deficiency (EFAD) who cannot maintain or restore a normal essential fatty acid pattern by oral intake.
Dosage/Direction for Use
Adult Max 3 g triglycerides/kg/day. Max infusion rate: 500 mL/5 hr. Neonate & infant 0.5-4 g triglycerides/kg/day. Max infusion rate: Not exceeding 0.17 g triglycerides/kg/hr. Premature & low birth wt neonate Continuous infusion over 24 hr. Initially 0.5-1 g/kg/day, may be increased by 0.5-1 g/kg/day up to 2 g/kg/day. Essential fatty acid deficiency 4-8% of nonprotein energy requirement to be supplied by Intralipid.
Contraindications
Hypersensitivity to egg, soya  or peanut protein. Severe hyperlipemia & liver insufficiency, acute shock, hemophagocytotic syndrome.
Special Precautions
Renal insufficiency, uncompensated DM, pancreatitis, impaired liver function (monitor liver function), hypothyroidism (if hypertriglyceridemic) & sepsis. Neonates & prematures w/ hyperbilirubinemia & pulmonary HTN. Platelet count, liver test & serum triglycerides conc should be monitored in neonates on long-term parenteral nutrition.
Adverse Reactions
Increase in body temp. Shivering, chills, nausea & vomiting.
Drug Interactions
Transient increase in lipolysis in plasma, resulting in a transient decrease in triglyceride clearance due to depletion of lipoprotein lipase w/ heparin.
