Granisetron HCl
Indications/Uses
Prevention or treatment of acute nausea & vomiting associated w/ chemo- & radiotherapy; post-op nausea & vomiting. Prevention of delayed nausea & vomiting associated w/ chemo- & radiotherapy.
Dosage/Direction for Use
May either be administered as a slow IV inj over 30 sec or as an IV infusion diluted in 20-50 mL infusion fluid over 5 min. Adult Prevention of acute/delayed chemo- & radiotherapy-induced nausea & vomiting (CINV & RINV) 1-3 mg (10-40 mcg/kg) prior to start of chemotherapy. Soln should be diluted to 5 mL per mg. Treatment of acute CINV/RINV 1-3 mg (10-40 mcg/kg). Max: 9 mg over 24 hr. Soln should be diluted to 5 mL per mg. Post-op nausea & vomiting 1 mg (10 mcg/kg) by slow IV inj. Max: 3 mg over 24 hr. Childn ≥2 yr Prevention of acute/delayed CINV & treatment of acute CINV 10-40 mcg/kg (up to 3 mg) administered as an IV infusion, diluted in 10-30 mL infusion fluid over 5 min prior to start of chemotherapy. One additional dose may be administered at least 10 min after the initial dose (w/in 24-hr period) if required.
Special Precautions
May reduce lower bowel motility. Monitor patients w/ signs of sub-acute intestinal obstruction. Reports of ECG changes including QT interval prolongation. Caution in patients w/ cardiac co-morbidities, on cardiotoxic chemotherapy &/or w/ concomitant electrolyte abnormalities. Reports of cross-sensitivity between 5-HT3 antagonists (eg, dolasetron, ondansetron). Patients on a controlled Na diet. Concomitant use w/ other serotonergic &/or neuroleptic drugs. Discontinue treatment if serotonin syndrome occurs. Avoid during pregnancy. Advisable not to breast-feed during treatment.
Adverse Reactions
Headache; constipation. Insomnia; diarrhoea; elevated hepatic transaminases.
Drug Interactions
Possible additive effect on QT interval w/ medicinal products known to prolong QT interval &/or which are arrhythmogenic. Reports of serotonin syndrome w/ 5-HT3 antagonists & other serotonergic medicinal products (including SSRIs & SNRIs).
