Indications and Dosage
Intravenous
Susceptible infections
Adult: 1 g or 2 g 12 hourly given via IV inj over 3-5 minutes or IV infusion over 20-30 minutes. Dosage recommendations may vary among countries and individual products (refer to detailed product or local treatment guidelines).
Renal Impairment
Haemodialysis patients: Loading: 1 g or 2 g. Maintenance: 0.5 g or 1 g once daily; give half-dose after each dialysis session.
CrCl (mL/min) Dosage
≤20 Loading: 1 g or 2 g. Maintenance: 0.5 g or 1 g once daily.
>20-50 Loading: 1 g or 2 g. Maintenance: 0.5 g or 1 g 12 hourly.

Contraindications
Hypersensitivity to cefpirome or other cephalosporins; history of hypersensitivity to other β-lactam antibacterial agents.
Special Precautions
Patient with history of allergic symptoms (including asthma, hives, rash). Renal impairment. Elderly. Pregnancy and lactation.
Adverse Reactions
Significant: Blood dyscrasias (e.g. anaemia, neutropenia, pancytopenia, thrombocytopenia, agranulocytosis); increased INR; hypersensitivity reactions (e.g. anaphylaxis); fungal or bacterial superinfection (prolonged use).
Gastrointestinal disorders: Nausea, vomiting, abdominal pain, diarrhoea.
General disorders and administration site conditions: Pain at inj site, fatigue.
Investigations: Increased serum AST or ALT.
Nervous system disorders: Dizziness, headache, seizure.
Monitoring Parameters
Perform culture and susceptibility tests; consult local institutional recommendations before treatment initiation due to antibiotic resistance risks. Monitor renal function and CBC with differential.
Drug Interactions
Reduced clearance with probenecid.
Lab Interference
May cause positive Coombs test and false-positive serum or urine creatinine with Jaffe reaction.
Action
Description:
Mechanism of Action: Cefpirome binds to 1 or more of the penicillin-binding proteins (PBPs) which inhibits the final transpeptidation step of peptidoglycan synthesis in the bacterial cell wall. This results in the inhibition of cell wall biosynthesis, leading to cell lysis and death.
Pharmacokinetics:
Distribution: Widely distributed into body tissues and fluids. Enters breast milk. Volume of distribution: 17.7-18.7 L. Plasma protein binding: 8.2-11.7%.
Excretion: Via urine (75.4-99.9% as unchanged drug). Elimination half-life: 1.71-2.3 hours.
