Atropine sulfate
Indications/Uses
Reduction of saliva secretion before general anaesth. Restores normal heartbeat during cardiac arrest. Antidote to some insecticides & in mushroom poisoning. In combination w/ other drugs to reverse the effect of muscle relaxants used during surgery.
Dosage/Direction for Use
Pre-op medication Adult IV 300-600 mcg immediately before anaesth induction. IM or SC 300-600 mcg 1 hr before anaesth induction. Childn SC Administer 30 min before anaesth induction. Childn 41 kg 600 mcg, 32 kg 500 mcg, 20-27 kg 400 mcg, 12-16 kg 300 mcg, 7-9 kg 200 mcg, <3 kg 100 mcg, premature infant 65 mcg. If IM route, administer 30-60 min before anaesth induction. Childn >1 yr Alternative dosage: 10-20 mcg/kg 30-60 min before anaesth induction. Antidote to cholinesterase inhibitors Adult 2 mg preferably IV. Childn 50 mcg/kg IV or IM, repeat dose every 5-10 min until signs of atropinisation appear. Antidote to organophosphate pesticides & in mushroom poisoning Adult 2 mg IV or IM. Childn 50 mcg/kg IV or IM, repeat dose every 10-30 min until muscarinic signs & symptoms subside. Reversal of effects of non-depolarising muscle relaxants Adult 0.6-1.2 mg IV in conjunction w/ neostigmine methylsulfate. CPR Adult 3 mg IV once. Childn 20 mcg/kg IV once. Bradycardia, particularly if complicated by hypotension Initially 300 mcg IV, increase to 1 mg if necessary.
Contraindications
Hypersensitivity. Closed-angle glaucoma; enlarged prostate; myasthenia gravis; pyloric stenosis; paralytic ileus; ulcerative colitis.
Special Precautions
Urinary difficulties; heart failure; heart attack; heart transplant; COPD; overactive thyroid; high BP; fever; diarrhoea; reflux oesophagitis. Pregnancy & lactation.
Adverse Reactions
Drowsiness, blurred vision, dry mouth w/ difficulty swallowing, thirst, dilation of the pupils, flushing, dryness of the skin, slow heart beat followed by fast heartbeat, palpitations, difficulty in passing urine or constipation; vomiting; confusion especially in the elderly or severe confusion; coughing & bringing up phlegm (bronchial plugs); restlessness; hallucinations; arrhythmias; AV block after heart transplantation; fever.
Drug Interactions
Antipsychotics or antidepressants; amantadine; antihistamines; disopyramide, mexiletine; ketoconazole; medicines that need to be dissolved slowly in the mouth.
