Amitriptyline HCl
Indications/Uses
Symptomatic treatment of depression. Relief of bed-wetting at night by childn ≥6 yr.
Dosage/Direction for Use
Depression Adult 50-75 mg daily in divided doses or as single dose at night, increasing to 150-200 mg daily. Maintenance: 50-100 mg at night to lessen chance of relapse. Adolescent & elderly 25-50 mg daily in divided doses or as single dose at night. Maintenance: 25-50 mg. Nightly bed-wetting Childn 11-16 yr 25-50 mg at night for not >3 mth; 6-10 yr 10-20 mg at night for not >3 mth; <6 yr Not recommended.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to amitriptyline, or to other TCAs. Hypersensitivity to sunset yellow (E110) (25-mg tab only). Heart disease; mania; severe liver disease. Concomitant use w/ MAOIs or use w/in the previous 14 days for depression. Lactation. Childn <6 yr.
Special Precautions
Thoughts of suicide & worsening of depression or anxiety disorder. QT interval prolongation. Blood disorders; psychiatric disorders; liver or CV disease; urinary retention or enlarged prostate gland; overactive thyroid gland & concomitant use w/ medicines to treat a thyroid disorder; history of epilepsy; electroconvulsive therapy; glaucoma; any surgery including dental, that involves an anaesth. Intolerance to some sugars. May impair alertness. Should not be taken in the 1st 3 mth & last 3 mth of pregnancy. Elderly.
Adverse Reactions
Prolonged QT interval. Purpura; disturbances in sexual function or sex drive, breast swelling in men & women, swelling of the testicles, production or over-production of breast milk, changes in blood sugar levels, increased appetite & wt gain; inappropriate secretion of antidiuretic hormone; dizziness, tiredness or sleepiness, weakness, headache, difficulty concentrating, confusion, difficulty sleeping, nightmares, slight hyperactivity, exaggerated behaviour, delusions, hallucinations, anxiety, excitement, disorientation, restlessness, nerve damage, pins & needles, lack of coordination, loss of control of or shaky movements, tremor, fits; postural hypotension, increased BP, fast/racing heart, palpitations, heart attack, irregular or slow heart-beats & very low BP, other changes in ECGs; feeling or being sick, diarrhoea, loss of appetite, inflammation of the mucus membranes in the mouth, swollen saliva glands, abdominal pains, black tongue, impairment of taste; changes in liver function, jaundice; increased sweating, hair loss, ringing in the ears, increased need to urinate; increased risk of bone fractures.
Drug Interactions
Altretamine; apraclonidine, brimonidine; baclofen; disulfiram; painkillers eg, nefopam, tramadol, codeine, dihydrocodeine; amiodarone, diltiazem, disopyramide, procainamide, propafenone, quinidine, sotalol, verapamil; glyceryl trinitrate, isosorbide dinitrate; rifampicin, linezolid, fluconazole; carbamazepine, phenobarb; terfenadine; methylphenidate; antihypertensives eg, guanethidine, debrisoquine, bethanidine or clonidine, diuretics; clozapine, pimozide, thioridazine, chlorpromazine, haloperidol, prochlorperazine, sulpiride; cimetidine, ethchlorvynol; OC; sibutramine; sympathomimetics, ritonavir; anaesth; anticholinergics; antidepressants. Avoid alcohol.
