Indications/Uses
Treatment of HTN; angina pectoris; heart rhythm disorders. Prevention of another heart attack.
Dosage/Direction for Use
Adult HTN 100 mg daily. Angina pectoris 100 mg daily, possibly divided in 2 doses of 50 mg. Heart rhythm disorders Maintenance: 50-100 mg daily. Prevention of another heart attack following initial IV treatment 50 mg 10 min after end of IV treatment, followed by 50 mg after 12 hr. Then 100 mg daily, possibly divided in 2 doses of 50 mg for 6 days or until hospital discharge. Patient w/ renal impairment or elderly Start w/ a lower dose.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Bradycardia; shock due to reduced heart activity; hypotension; metabolic acidosis; severe blood flow disorder; 2nd or 3rd degree heart block; sick sinus syndrome; untreated pheochromocytoma; uncontrolled heart failure.
Special Precautions
Patients w/ untreated decompensation cordis; Prinzmetal angina; peripheral blood flow disorders; 1st degree heart block; DM; hypersensitivity to certain substances; disorder of the airways; renal impairment. Reduce dose when heart beat drops to <50-55 bpm. Gradually reduce dose over 1-2 wk when stopping treatment. Can cause dizziness which may affect ability to drive or operate machines. Pregnancy & lactation. Not recommended in childn.
Adverse Reactions
Bradycardia, cold limbs or blue coloring of the limbs; nausea, vomiting, non-severe diarrhoea; fatigue, sweating.
Drug Interactions
Increased risk of hypotension, bradycardia or heart failure w/ Ca antagonists. Increased effectiveness & increased risk of heart failure w/ dihydropyridine derivatives. Concomitant use w/ digitalis glycosides or medicinal products that have a stabilizing effect on heart rhythm (eg, disopyramide & quinidine) can have an effect on the heart. Use of clonidine should only be stopped a few days after atenolol is stopped. Reduced effect w/ medicinal products that have a stimulating effect on the nervous system eg, adrenaline; certain analgesics, antipyretics & anti-inflammatory eg, ibuprofen & indomethacin. Increased effect of oral antidiabetics & insulin. Increased BP-lowering effect w/ other BP-lowering medicines. Potential interaction w/ inhalation anaesth.
