Metronidazole
Indications/Uses
Asymptomatic & symptomatic trichomoniasis, infections due to susceptible anaerobic bacteria, intestinal & hepatic amoebiasis, & giardiasis.
Dosage/Direction for Use
Trichomoniasis Adult 2 g as a single dose, or as a 2-day course of 800 mg in the morning & 1.2 g in the evening, or as 7-day course of 600 mg-1 g daily in 2 or 3 divided doses. Anaerobic bacterial infections Initially 800 mg followed by 400 mg every 8 hr for 7 days. Amoebiasis Adult 400-800 mg tid for 5-10 days. Alternatively, 1.5-2.5 g as a single daily dose for 2-3 days. Childn 7-10 yr 1/2 of the total adult daily dose. Alternatively, 35-50 mg/kg in divided doses, 3-7 yr 1/3 of the total adult daily dose, 1-3 yr 1/4 of the total adult daily dose. Giardiasis Adult 2 g once daily for 3 successive days, or 400 mg tid for 5 days, or 500 mg bid for 7-10 days. Childn 15 mg/kg daily in divided doses.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to metronidazole or other nitroimidazole derivatives. History or evidence of blood dyscrasia. Trichomoniasis: Pregnancy (1st trimester).
Special Precautions
Monitor for neurological sign. Candida overgrowth may occur. Concomitant use w/ alcohol & warfarin. Avoid during pregnancy. Not recommended during lactation.
Adverse Reactions
GI disturbances especially nausea & unpleasant metallic taste; vomiting, diarrhea, constipation. Weakness, dizziness, ataxia, headache, drowsiness, insomnia, & change in mood or mental state eg, depression or confusion. Peripheral neuropathy (usually as numbness or tingling in the extremities) & epileptic seizures w/ high doses or prolonged treatment. Temporary moderate leucopenia, thrombocytopenia; urethral discomfort, darkening of the urine.
Drug Interactions
May provoke a disulfiram-like reaction w/ alcohol. Acute psychosis or confusion w/ disulfiram. Impaired metabolism or excretion of warfarin, phenytoin, lithium, ciclosporin, & fluorouracil. Decreased plasma conc w/ phenobarb. Increased plasma conc & might increase risk of neurological adverse effects w/ cimetidine.
