Indications/Uses
As monotherapy or in combination w/ other antihypertensive medications for essential HTN.
Dosage/Direction for Use
Adult 10-20 mg once daily. Elderly Start at a low dose ie, 5 mg & titrate upwards according to response.
Administration
Should be taken with food: Do not crush/chew/dissolve.
Contraindications
Hypersensitivity. Women who may be pregnant. Pregnancy & lactation.
Special Precautions
Patients w/ hypotension, poor cardiac reserve & heart failure, severe liver dysfunction, or history of severe adverse effects due to Ca antagonists. May exacerbate angina w/ sudden w/drawal. Discontinue in patients who experience ischemic pain following administration. False elevated spectrophotometric values of urinary vanillylmandelic acid may occur. Do not drive a vehicle or operate a machine or perform tasks that require alertness if experiencing syncope, dizziness, hypotension, visual disturbances & blurred vision. Childn.
Adverse Reactions
Dizziness, flushing, headache, hypotension, peripheral edema, tachycardia, palpitations, GI disturbances. Increased frequency of micturition, lethargy, eye pain, depression, ischemic chest pain, cerebral or myocardial ischemia, transient blindness. Rashes, fever, abnormal liver function, gingival hyperplasia, myalgia, tremor, impotence. Liver dysfunction accompanied by elevated AST (SGOT), ALT (SGPT), γ-GTP, & jaundice. Thrombocytopenia. Elevated LDH; creatinine, elevation of urea nitrogen, urine protein +ve; shoulder stiffness; facial flushing, heat sensation, ECG abnormality (ST decline, T wave reversal), decrease BP; nausea, vomiting, abdominal pain; variation of WBC count, neutrophils, Hb; edema (face, lower limb), general malaise, frequent urination, elevation of serum cholesterol, changes in CK (CPK), uric acid, serum K, serum P.
Drug Interactions
May interact w/ other antihypertensive agents, aldesleukin, & antipsychotics which may cause hypotension. May modify insulin & glucose responses. Interactions may occur w/ quinidine, carbamazepine, phenytoin, & erythromycin. Increased conc & reduced extrarenal clearance of digoxin. May enhance antihypertensive effect w/ cimetidine; azole antifungal agents ie, itraconazole, miconazole; grapefruit juice. Attenuated antihypertensive effect & may promote metabolism & clearance w/ rifampicin.
