Meropenem
Indications/Uses
Pneumonia including nosocomial pneumonia; UTI; gynecological, skin & skin-structure infections; meningitis, septicaemia. Empiric treatment for presumed infections w/ febrile neutropenia as a single agent or in combination w/ antiviral or antifungal agents. Polymicrobic infections as a single agent or in combination w/ other antibiotics.
Dosage/Direction for Use
Adult Pneumonia, UTI, gynaecological, skin & skin-structure infections 500 mg IV 8 hrly. Nosocomial pneumonia, peritonitis, presumed infections in febrile neutropenia, septicaemia 1 g IV 8 hrly Meningitis 2 g 8 hrly. Renal impairment CrCl 26-50 mL/min 1 unit dose 12 hrly, CrCl 10-25 mL/min ½ unit dose 12 hrly, CrCl <10 mL/min ½ unit dose 24 hrly. Childn>3 mth-12 yr 10-20 mg/kg 8 hrly, >50 kg Adult dose. Childn Meningitis 40 mg/kg 8 hrly.
Contraindications
Hypersensitivity to meropenem or anaphylactic reactions to β-lactams.
Special Precautions
Monitor of transaminase & bilirubin levels in patients w/ hepatic disease. Overgrowth of non-susceptible organisms may occur. Not recommended for use in methicillin-resistant staphylococci infections. Pseudomembranous colitis. History of GI complaints. Caution in co-administration w/ potentially nephrotoxic drugs. Not recommended in infant <3 mth. Pregnancy & lactation.
Adverse Reactions
Inflammation, thrombophlebitis, pain at the site of inj. Rash, pruritus, urticaria; rare severe skin reactions eg, erythema multiforme, Stevens-Johnson syndrome & toxic epidermal necrolysis. Abdominal pain, nausea, vomiting, diarrhoea, pseudomembranous colitis. Reversible thrombocythaemia, eosinophilia, thrombocytopenia, leucopenia & neutropenia; +ve direct or indirect Coombs test may develop; reduced partial thromboplastin time. Increased serum conc of bilirubin, transaminases, alkaline phosphatase & lactic dehydrogenase. Headache, paraesthesiae. Oral & vaginal candidiasis.
Drug Interactions
Increased plasma conc w/ probenecid. May reduce serum valproic acid levels.
