Indications/Uses
Moderate to severe atopic dermatitis in patients who are not adequately responsive to or are intolerant of conventional therapies eg, topical corticosteroids.
Dosage/Direction for Use
Adult & adolescent ≥16 yr Flare treatment Apply 0.1% oint bd to the affected area. Restart bd treatment w/ 0.1% oint if symptoms recur. Maintenance treatment Apply 0.1% oint once daily to the affected area twice wkly w/ 2-3 days interval. Re-initiate bd treatment w/ 0.1% oint if signs of flare recur. Childn 2-16 yr Use 0.03% oint.
Contraindications
Hypersensitivity to tacrolimus or macrolides.
Special Precautions
Not to be applied to potentially malignant or pre-malignant lesions. Not recommended in patients w/ skin barrier defect eg, Netherton's syndrome, lamellar ichthyosis, generalized erythroderma or cutaneous Graft Versus Host disease. Not to be used in patients w/ congenital or acquired immunodeficiencies or on therapy that causes immunosuppression. Exercise care in patients w/ extensive skin involvement over an extended period of time, especially in childn. Clear clinical infections at treatment sites prior to treatment. Minimise skin exposure to sunlight & avoid use of UV light from solarium, therapy w/ UVB or PUVA. Monitor patients who develop lymphadenopathy. Avoid contact w/ eyes & mucous membranes. Not recommended under occlusive dressings. Not to be applied on the same area w/ emollients w/in 2 hr of application. Hepatic failure. Not to be used in childn <2 yr.
Adverse Reactions
Application site burning & pruritus. Local skin infection eg, eczema herpeticum, folliculitis, herpes simplex & virus infection, Kaposi's varicelliform eruption; alcohol intolerance; paraesthesias, dysaesthesias; pruritus; application site warmth, erythema, pain, irritation, paraesthesia & rash.
Drug Interactions
Concomitant systemic administration of CYP3A4 inhibitors eg, erythromycin, itraconazole, ketoconazole, diltiazem.
