K citrate
Renal tubular acidosis (RTA) w/ Ca stones; hypocitraturic Ca oxalate nephrolithiasis of any etiology; uric acid lithiasis w/ or w/o Ca stones. Prevention of stone relapse after extracorporeal shock wave lithotripsy (ESWL) or lithotomy.
Dosage/Direction for Use
Adult Severe hypocitraturia (urinary citrate <150 mg/day) 6 tab daily in 3 divided doses. Mild to moderate hypocitraturia (urinary citrate >150 mg/day) 3 tab daily in 3 divided doses.
Should be taken with food: Take 30 min after meals.
Patients w/ conditions predisposing to hyperkalemia eg, chronic renal failure, uncontrolled DM, familial hyperkalemic periodic paralysis (adynamia episodica), acute dehydration, strenuous physical exercise in unconditioned individuals, adrenal insufficiency, extensive tissue breakdown, severe myocardial damage, administration of K-sparing diuretic. Patient w/ slow gastric emptying, esophageal compression, intestinal obstruction or stricture. Patients on anticholinergic therapy. Active PUD. Active UTI. Renal insufficiency GFR <0.7 mL/kg/min. Resp or metabolic acidosis.
Special Precautions
Avoid in chronic renal failure or impaired K secretion eg, severe myocardial damage or heart failure can produce hyperkalemia & cardiac arrest. Monitor serum K regularly. Determine periodically acid-base balance, other serum electrolyte levels, & ECG. Discontinue if hyperkalemia, a significant rise in serum creatinine or a significant fall in blood hematocrit or Hb, vomiting, abdominal pain or GI bleeding occurs. Pregnancy & lactation.
Adverse Reactions
Abdominal discomfort, vomiting, diarrhea, loose bowel movements & nausea.
Drug Interactions
May produce severe hyperkalemia w/ K-sparing diuretics (eg, triamterene, spironolactone or amiloride). Concomitant use w/ anticholinergics increase GI irritation.
