Sitagliptin 50 mg, metformin HCl 500 mg
As adjunct to diet & exercise to improve glycemic control in adults w/ type 2 DM.
Dosage/Direction for Use
Individualized dosage. Initially 50 mg sitagliptin/500 mg metformin bd w/ gradual dose escalation. Max daily dose: 100 mg sitagliptin & 2,000 mg metformin. Patient already treated w/ metformin 50 mg sitagliptin bd (100 mg total daily dose) & dose of metformin already being taken. Patient taking metformin 850 mg bd 50 mg sitagliptin/1,000 mg metformin bd.
Should be taken with food.
Hypersensitivity. Diabetic ketoacidosis & pre-coma. Moderate & severe renal impairment (CrCl <60 mL/min).
Special Precautions
Discontinue & use insulin during periods of stress & decreased intake of fluids & food. Assess renal function prior to initiating treatment & at least annually thereafter. Not to be used in hepatic disease. Pregnancy. Not to be used during lactation. Elderly. Ped <18 yr.
Adverse Reactions
Diarrhea, nausea/vomiting, flatulence, abdominal discomfort, indigestion, asthenia, & headache.
Drug Interactions
Potential interaction by competing for common renal tubular transport systems w/ cationic drugs eg, amiloride, digoxin, morphine, procainamide, quinidine, quinine, ranitidine, triamterene, trimethoprim, or vancomycin. Sitagliptin: May slightly increase mean peak drug conc of digoxin.
