Risperidone
Indications/Uses
Acute & chronic schizophrenia, affective symptomatology of schizophrenia & other psychoses.
Dosage/Direction for Use
Adult 1 mg bd on day 1, 2 mg bd on day 2 & 3 mg bd on day 3. Stabilize for 1 wk. Adjust dose further after 1 wk by 1-mg increment bd up to a max of 8 mg/day. Elderly 0.5 mg bd on day 1, increase by 0.5 mg to achieve 1-2 mg bd. Discontinue if tardive dyskinesia occurs.
Administration
May be taken with or without food.
Contraindications
Special Precautions
AMI, IHD, heart failure, conduction abnormality, treatment w/ antihypertensive medications, conditions pre-disposing to hypotension eg, dehydration, hypovolemia, risperidone. Epilepsy, hyperprolactinemia, parkinsonism, old age, renal & hepatic impairment. Monitor neuroleptic malignant syndrome & tardive dyskinesia development & w/draw risperidone if these occur. Occasional QT interval prolongation. Pro-arrhythmic effect. Pregnancy & lactation. Childn <15 yr.
Adverse Reactions
Extrapyramidal symptoms (exclusively in doses >6 mg/day), tremor, rigidity, hypokinesia, dystonia, ataxia/gait disturbance, dizziness, constipation, anxiety, somnolence, rash, rhinitis, tachycardia.
Drug Interactions
Increased clearance w/ CNS active drugs, levodopa, antihypertensive agents, alcohol, carbamazepine. Decreased clearance w/ clozapine.
