Each 250- and 500-mg tablet contains ciprofloxacin HCl equivalent to ciprofloxacin 250 mg and 500 mg, respectively.
Fluoroquinolone antibiotic.
Pharmacology: Ciprofloxacin is bactericidal and acts by inhibiting the A subunit of DNA gyrase (topoisomerase) which is essential in the reproduction of bacterial DNA.
Ciprofloxacin is highly active in vitro against most gram-negative aerobic bacteria including Enterobacteriaceae, Haemophilus, Neisseriae and Pseudomonas aeruginosa. Ciprofloxacin is also active in vitro against many gram-positive aerobic bacteria, including penicillinase- and nonpenicillinase-producing and methicillin-resistant staphylococci. Many strains of streptococci are relatively resistant to ciprofloxacin. The in vitro antimicrobial activity of ciprofloxacin is not affected by the presence of pus.
Pharmacokinetics: Ciprofloxacin is rapidly absorbed, Cmax ranges between 1.5-2 mg/L with tmax of about 60-75 min. It is widely distributed into body fluids and tissues except CSF, metabolised in the body and eliminated mainly by kidney. Half-life is about 4 hrs. Ciprofloxacin crosses into foetal blood and is excreted into breast milk.
Treatment of: Urinary tract infections (UTI); enteric infections; bone and joint infections; sexually transmitted diseases (STD); respiratory tract infections (RTI). Prophylaxis against infections in granulocytopenic patients.
250-750 mg twice daily orally, depending on the severity and nature of infection. Dose adjustment is required in patients with renal impairment. A single oral dose of 500 mg is suggested for the treatment of gonorrhoea and prophylaxis of meningococcal meningitis.
In the event of acute overdosage, the stomach should be emptied by inducing vomiting or by gastric lavage. Adequate hydration should be maintained; only a small amount of ciprofloxacin is removed from the body after hemodialysis or peritoneal dialysis.
Hypersensitivity to quinolones.
Use in pregnancy & lactation: Since ciprofloxacin causes arthropathy in immature animals, it should not be used in pregnant and lactating women.
Use in children: Ciprofloxacin should not be used in children <15 years.
Ciprofloxacin should be used with caution in patients with suspected CNS disorders eg, cerebral arteriosclerosis, epilepsy. Patients receiving ciprofloxacin should be well hydrated. Crystalluria may occur in a neutral or alkaline pH. Caution should be exercised during concurrent administration of antacids, theophylline and probenecid in patients with renal impairment.
Since ciprofloxacin causes arthropathy in immature animals, it should not be used in pregnant and lactating women.
Ciprofloxacin is generally well tolerated. Adverse effects are generally mild which include gastrointestinal disturbances, skin reactions, CNS effects, and renal, hematological and hepatic disorders as well as allergic reactions.
Store in a cool and dry place.
