Indications and Dosage
Ophthalmic
Ocular hypertension, Open-angle glaucoma
Adult: As 0.1% or 0.3% drops: Instill 1 drop bid into the affected eye.
Contraindications
Bronchial asthma or history of bronchial asthma, severe COPD, symptomatic sinus bradycardia, 2nd or 3rd degree AV block, cardiogenic shock, overt cardiac failure.
Special Precautions
Patients w/ myasthenia gravis, a history of cardiac failure or psychiatric disease, DM, cerebrovascular insufficiency, peripheral vascular disease including Raynaud's syndrome. Avoid abrupt withdrawal in patients undergoing elective surgery. May mask certain signs and symptoms of hyperthyroidism. Pregnancy and lactation.
Adverse Reactions
Transient local discomfort, abnormal vision, conjunctivitis, conjunctival leucoplakia, blepharitis, blurred vision, browache, oedema, eyelid dermatitis, photophobia, tearing, uveitis; hypersensitivity reaction, rash, angina, atrial fibrillation, MI, palpitation, bradycardia, anxiety, depression, HTN, dizziness, headache, nervousness, somnolence, asthenia, arthritis, myalgia, bronchitis, cough, dyspnoea, rhinitis, epistaxis, nausea.
Patient Counseling Information
Metipranolol contains benzalkonium Cl which may be absorbed by soft contact lenses. Remove lens prior to admin and wait 15 min before reinserting.
Drug Interactions
Potential additive effects on systemic β-blockade w/ oral β-blockers. Possible additive hypotensive effects or bradycardia w/ catecholamine-depleting drugs (e.g. reserpine). Possible precipitation of left ventricular failure w/ Ca channel blockers. Concomitant use w/ digitalis and Ca channel blockers may cause additive prolonging effects on AV conduction time.
Action
Description: Metipranolol is a non-cardioselective β-blocker w/o significant intrinsic sympathomimetic activity and membrane-stabilising properties. It has weak local anaesthetic and myocardial depressant activity. It reduces intraocular pressure appears to be via decreased production of aqueous humour.
Onset: ≤30 min.
Duration: Intraocular pressure reduction: Up to 24 hr.

Store between 15-30°C.
