Methylcobalamin 1,500 mcg, pregabalin 150 mg
Indications/Uses
Painful neuropathies eg, diabetic neuropathy, post-herpetic neuralgia, trigeminal neuralgia, following nerve entrapment as in prolapsed intervertebral disc, complex regional pain syndrome, infective neuropathies, HIV sensory neuropathy, alcoholic neuropathy, nutritional neuropathies, phantom limb pain.
Dosage/Direction for Use
Adult 1 tab once daily. Max: 600 mg/day. Increase dose as necessary to 1 tab bd. Patient w/ on-going pain ≥300 mg/day. Renal dysfunction CrCl 30-60 mL/min Max: 300 mg/day. CrCl 15-30 mL/min Max: 150 mg/day. CrCl <15 mL/min max: 75 mg/day.
Administration
May be taken with or without food.
Special Precautions
Patient w/ heart failure & diabetes for the possibility of edema & wt gain. Diabetic patients may develop skin ulcerations. Patients w/ early Leber's disease when treated w/ vit B12 can suffer from severe & swift optic atrophy. Do not discontinue abruptly due to the possibility of increasing seizure frequency. It must be done gradually over 1 wk. Seek medical attention if vision disturbances & unexplained muscle pain, tenderness or weakness accompanied by malaise or fever occur. Advocate w/ caution along w/ thiazolidinediones in view of wt gain & fluid retention. Do not take w/ alcohol or combine w/ other CNS depressants. May affect ability to drive & operate complex machinery. Renal dysfunction. Pregnancy & lactation.
Adverse Reactions
Methylcobalamin: pulmonary edema & CHF early in treatment, peripheral vascular thrombosis; polycythemia vera; mild transient diarrhea; itching, transitory exanthema; feeling of swelling of entire body. Pregabalin: dizziness, somnolence, dry mouth, edema, blurred vision, wt gain, "thinking abnormal" (primarily difficulty w/ concentration & attention); asthenia, ataxia, abnormal gait, paresthesias, diplopia, blurred vision, visual field defect, nystagmus, nausea, tremor, vertigo, headache, nervousness, confusion & edema. Abdominal pain, allergic reaction, fever, flu syndrome, accidental injury; ST depressed, ventricular fibrillation; gastroenteritis, nausea, diarrhea, flatulence, dry mouth, increased appetite, constipation; ecchymosis; wt gain, peripheral edema; arthralgia, leg cramps; myalgia, myasthenia; headache, nervousness, dizziness, vertigo, somnolence, ataxia, tremor, thinking abnormal, amnesia, speech disorder, incoordination, abnormal gait, twitching, confusion, anxiety, depersonalization, hypertonia, hypesthesia, libido decreased, nystagmus, paresthesia, stupor, twitching, euphoria; bronchitis, dyspnea; pruritus, face swelling, Stevens-Johnson syndrome; conjunctivitis, diplopia, blurred vision, abnormal vision, otitis media, tinnitus; anorgasmia, impotence, urinary frequency, urinary incontinence.
Drug Interactions
Gabapentin may slightly decrease the rate of absorption of Pregabalin when administered together.
