Linezolid
Indications/Uses
Nosocomial & community acquired pneumonia. Complicated skin & soft tissue infections.
Dosage/Direction for Use
Adult 600 mg bd for 10-14 consecutive days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Uncontrolled HTN, phaeochromocytoma, carcinoid, thyrotoxicosis, bipolar depression, schizoaffective disorder, acute confusional states. Concomitant use w/ MAOIs (or w/in 2 wk of such therapy); SSRIs, TCAs, serotonin 5-HT1 receptor agonists, directly & indirectly acting sympathomimetic agents, vasopressives, dopaminergics, pethidine or buspirone. Lactation.
Special Precautions
Monitor myelosuppression. Patients w/ preexisting anaemia, granulocytopenia or thrombocytopenia; concomitant use w/ medications decreasing Hb, depress blood counts or adversely affect platelet count or function. Weekly monitor complete blood counts (including Hb levels, platelets & total & differentiated leukocyte counts). Mortality imbalance in patients w/ catheter-related gm +ve bloodstream infections. Risk of antibiotic-associated diarrhoea & colitis (including pseudomembranous colitis & Clostridium difficile-associated diarrhea); lactic acidosis; mitochondrial dysfunction; serotonin syndrome; peripheral & optic neuropathy; convulsions; superinfection. Excessive consumption of tyramine-rich foods. Do not drive or operate machinery if dizziness or visual impairment occurs. Patients w/ severe hepatic & renal insufficiency (including those undergoing dialysis). Pregnancy. Childn <18 yr.
Adverse Reactions
Candidiasis, oral candidiasis, vag candidiasis, fungal infections; anaemia; insomnia; headache, taste perversion (metallic taste), dizziness; HTN; diarrhoea, nausea, vomiting, localised or general abdominal pain, constipation, dry mouth, dyspepsia; abnormal liver function test, increased AST, ALT or alkaline phosphatase; pruritus, rash; increased BUN; fever, localised pain; increased LDH, creatine kinase, lipase, amylase or non-fasting glucose; decreased total protein, albumin, Na or Ca; increased or decreased K or bicarbonate.
Drug Interactions
MAOIs. Increased BP caused by pseudoephedrine & phenylpropanolamine HCl. Potential serotonergic interaction w/ dextromethorphan. Avoid excessive amounts of tyramine-rich foods. Decreased Cmax & AUC w/ rifampicin. Reduced mean max & AUC INR w/ warfarin.
