Indications/Uses
Management of neuropathic pain associated w/ diabetic peripheral neuropathy, postherpetic neuralgia, fibromyalgia. Adjunctive therapy for adults w/ partial onset seizures.
Dosage/Direction for Use
Management therapy: Neuropathic pain associated w/ diabetic peripheral neuropathy Initially, 50 mg tds (150 mg/day), may be increased to 300 mg/day w/in 1 wk. Patient w/ CrCl of at least 60 mL/min Max: 100 mg tds (300 mg/day). Postherpetic neuralgia Initially, 75 mg bd, or 50 mg tds (150 mg/day), may be increased to 300 mg/day w/in 1 wk. May be increased further to 300 mg bd or 200 mg tds in patients who do not achieve sufficient pain relief after 2-4 wk, & can tolerate the treatment. Patient w/ CrCl of at least 60 mL/min 75-150 mg bd, or 50-100 mg tds. Fibromyalgia Initially 75 mg bd (150 mg/day), may be increased to 150 mg bd (300 mg/day) w/in 1 wk. May be increased further to 225 mg bd (450 mg/day). Adjunctive therapy of partial onset seizures 75 mg bd or 50 mg tds (total daily dose: ≤150 mg/day). Max: 600 mg/day. Renal impairment: CrCl 30-60 mL/min Total daily dose: 75 mg/day, 150 mg/day, 225 mg/day, or 300 mg/day, bd or tds; 15-30 mL/min Total daily dose: 25-50 mg/day, 75 mg/day, 100-150 mg/day or 150 mg/day, as a single or in 2 divided doses; <15 mL/min Total daily dose: 25 mg/day, 25-50 mg/day, 50-75 mg/day or 75 mg/day once daily; Hemodialysis Adjust daily dose based on renal function & administer supplemental dose, immediately following 4 hrly hemodialysis treatment. Supplemental dose: 25 mg once daily regimen: 25 or 50 mg single dose; 25-50 mg once daily regimen: 50-75 mg single dose; 50-75 mg once daily regimen: 75 or 100 mg single dose; 75 mg once daily regimen: 100 or 150 mg single dose.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Hypersensitivity shortly after initiation may occur. Not recommended for doses >300 mg/day in management therapy for neuropathic pain associated w/ diabetic peripheral neuropathy & post herpetic neuralgia; >450 mg/day in management of fibromyalgia. Previous episode of angioedema; risk of developing angioedema may be increased in concomitant treatment w/ drugs associated w/ angioedema, eg, ACE inhibitors. Gradually w/draw treatment to prevent increased seizure frequency; avoid insomnia, nausea, headache & diarrhea w/ abrupt or rapid w/drawal, & taper gradually over a min of 1 wk instead. Increased risk of suicidal thoughts or behavior; monitor for emergence or worsening of signs & symptoms of depression, suicidal thoughts or behavior, &/or unusual changes in mood or behavior. Peripheral edema may occur; higher frequencies of wt gain & peripheral edema in concomitant use w/ thiazolidinedione antidiabetics; patients w/ CHF NYHA Class III or IV. May cause wt gain. Potentially, high incidence of hemangiosarcoma. Blurred vision may occur; assess patients if changes in vision occur; consider further assessment if visual disturbance persists; consider more frequent assessments in patients routinely monitored for ocular conditions. Creatine kinase elevation may occur; discontinue treatment for diagnosed or suspected myopathy or markedly elevated creatine kinase levels occur. May cause decreased platelet count, prolonged PR interval. Carefully evaluate patients w/ history of drug abuse & observe for signs of misuse or abuse. Potential dizziness & somnolence; may impair ability to drive or operate machinery. Renal impairment. Pregnancy & lactation. Childn. Elderly w/ renal impairment.
Adverse Reactions
Dizziness & somnolence frequently leading to discontinuation; headache, ataxia, asthenia, dry mouth, constipation, edema, blurred vision, wt gain, abnormal thinking, ie, difficulty w/ conc/attention.
Drug Interactions
Potentially, additive cognitive & gross motor functioning effects w/ oxycodone, lorazepam or ethanol.
