Azithromycin dihydrate
Indications/Uses
Infections caused by susceptible organisms in lower resp tract (eg, bronchitis, pneumonia), skin & soft tissue, upper resp tract (eg, sinusitis, pharyngitis/tonsillitis); acute otitis media. STD eg, uncomplicated genital infections due to Chlamydia trachomatis or nonmultiresistant Neisseria gonorrhoea & chancroid due to Haemophilus ducreyi. Alone or in combination w/ rifabutin for prophylaxis against Mycobacterium avium-intracellulare complex (MAC) infection & in combination w/ ethambutol for disseminated MAC (DMAC).
Dosage/Direction for Use
Adult & elderly Other indications Total dose: 1,500 mg; 500 mg daily for 3 days, or 500 mg on day 1 then 250 mg daily for days 2-5. STD caused by C trachomatis or susceptible N gonorrhoea & chancroid due to H ducreyi 1 g single dose. Prophylaxis against MAC infections in patient w/ HIV infections 1,200 mg once wkly. In combination w/ ethambutol for DMAC infections in patient w/ advanced HIV infection 600 mg once daily. Childn >45 kg Total dose: 30 mg/kg; 10 mg/kg once daily for 3 days, or 10 mg/kg on day 1 then 5 mg/kg on days 2-5. Streptococci pharyngitis Single dose of 10 mg/kg or 20 mg/kg for 3 days. Max: 500 mg daily.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity to macrolides. Co-administration w/ ergot derivatives.
Special Precautions
Clostridium difficile-associated diarrhea; significant hepatic diseases & severe renal impairment (GFR <10 mL/min), prolonged cardiac repolarisation & QT interval. Observe for signs of superinfection w/ non-susceptible organisms including fungi. Pregnancy & lactation.
Adverse Reactions
CNS & GI disturbances; interstitial nephritis & acute failure; thrombocytopenia; abnormal liver function; vaginitis.
Drug Interactions
Antacids, cyclosporin, digoxin, ergot derivatives, coumarin-type oral anticoagulants, zidovudine.
