In combination w/ metronidazole for adults w/ complicated intra-abdominal infections (cIAI). Adults w/ complicated UTI (cUTI) including pyelonephritis; nosocomial pneumonia including ventilator-associated pneumonia (VAP).
cIAI 1.5 g every 8 hr for 4-14 days in conjunction w/ metronidazole 500 mg IV every 8 hr. cUTI including pyelonephritis 1.5 g every 8 hr for 7 days. Renal impairmentCrCl 30-50 mL/min 750 mg IV every 8 hr, 15-29 mL/min375 mg IV every 8 hr. ESRD on hemodialysisSingle loading dose: 750 mg followed by maintenance dose: 150 mg every 8 hr. Nosocomial pneumonia including VAP3 g every 8 hr for 8-14 days. Renal impairment CrCl 30-50 mL/min 1.5 g IV every 8 hr, 15-29 mL/min 750 mg IV every 8 hr. ESRD on hemodialysis Single loading dose: 2.25 g followed by maintenance dose: 450 mg every 8 hr.
Hypersensitivity reactions to other cephalosporins, penicillins, carbapenems or other β-lactams. Discontinue use if Clostridium-difficile-associated diarrhea is confirmed or allergic reactions occurs. Risk of development of drug-resistant bacteria. Positive direct Coomb's test. Pregnancy & lactation. Ped. Elderly ≥65 yr.
