Alprazolam
Indications/Uses
Anxiety states (anxiety neuroses), mixed anxiety-depression, neurotic or reactive depression, panic-related disorders.
Dosage/Direction for Use
Anxiety Initially 0.75-1.5 mg daily in divided doses. Usual dosage range: 0.5-4 mg daily in divided doses. Depression Initially 1.5 mg daily in divided doses. Usual dosage range: 1.5-4.5 mg daily in divided doses. Panic disorders Initially 0.5-1 mg at bedtime or 0.5 mg tds. Usual dosage range: Adjusted according to patient's response in increments not >1 mg daily every 3-4 days. Elderly Initially 0.5-0.75 mg daily in divided doses, adjusted according to patient's response.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to benzodiazepines, alprazolam.
Special Precautions
Anaphylaxis & angioedema. Habituation & emotional/physical dependence. History of alcoholism or drug abuse. Abuse-prone individuals. Depressed or suicidal patients. Complex sleep-related behaviors. Avoid abrupt discontinuation due to w/drawal symptoms. Not recommended in concomitant use w/ ketoconazole, itraconazole, other azole-type antifungals. Concomitant use w/ nefazodone, fluvoxamine, cimetidine; fluoxetine, propoxyphene, OCs, diltiazem, macrolides. May affect ability to drive & use machines. Renal or hepatic impairment. Pregnancy. Not to be used during lactation. Childn <18 yr.
Adverse Reactions
Depression; sedation, somnolence, ataxia, memory impairment, dysarthria, dizziness, headache; constipation, dry mouth; fatigue, irritability. Decreased appetite; confusional state, disorientation, libido changes, anxiety, insomnia, nervousness; balance disorder, abnormal coordination, attention disturbance, hypersomnia, lethargy, tremor; blurred vision; nausea; dermatitis; sexual dysfunction; increased/decreased wt.
Drug Interactions
Increased risk of resp depression, profound sedation, coma & death w/ benzodiazepines. Co-administration w/ ketoconazole, itraconazole or other azole-type antifungals; nefazodone, fluvoxamine, cimetidine; fluoxetine, propoxyphene, OCs, diltiazem or macrolides eg, erythromycin & troleandomycin. Impaired clearance, prolonged elimination t½ & enhanced clinical effects w/ HIV PIs eg, ritonavir. Increased digoxin conc.
