Indications/Uses
Rocky Mountain spotted fever, typhus fever & group, Q fever, rickettsialpox & tick fevers. Resp infections, psittacosis, lymphogranuloma venereum, uncomplicated urethral, endocervical or rectal infections in adults, trachoma, inclusion conjunctivitis, granuloma inguinale (donovanosis), Lyme disease (stage 1), louse- or tick-borne relapsing fever, nongonococcal urethritis. Infections caused by susceptible gm -ve & +ve microorganisms. Adjunct treatment in acute intestinal amoebiasis & severe acne. Prophylaxis & treatment of malaria, leptospirosis & cholera. Prophylaxis in scrub typhus & traveler's diarrhea.
Dosage/Direction for Use
Adult 200 mg on 1st day, then 100 mg daily. Severe infection 200 mg daily. Childn >8 yr >45 kg Adult dose, ≤45 kg 4.4 mg/kg, followed by 2.2 mg/kg. More severe infection Max: 4.4 mg/kg. Tick- & louse-borne relapsing fever, louse-borne typhus 100 or 200 mg single oral dose or 100 mg every 12 hr for 7 days. Early Lyme disease 100 mg bd for 14-60 days. Uncomplicated urethral, endocervical or rectal infection Adult 100 mg bd for 7 days. Nongonococcal urethritis 100 mg bd for 7 days. Lymphogranuloma venereum 100 mg bd for min of 21 days. Uncomplicated gonococcal infection of cervix, rectum or urethra, pharynx 100 mg bd for 7 days + co-treatment w/ cephalosporin or quinolone. Primary & secondary syphilis 100 mg bd for 2 wk. Latent & tertiary syphilis 100 mg bd for 2-4 wk. Acute pelvic inflammatory disease In-patient: 100 mg every 12 hr + cephalosporin then continue 100 mg bd for 14 days. Outpatient: 100 mg bd for 14 days + cephalosporin & probenecid. Acne vulgaris 50-100 mg daily for up to 12 wk. Chloroquine-resistant falciparum malaria 200 mg daily for at least 7 days. Prophylaxis of malaria Adult 100 mg daily, childn >8 yr 2 mg/kg daily. Prophylaxis of cholera Adult 300 mg single dose. Prevention of scrub typhus 200 mg single dose. Prevention of traveler's diarrhea 200 mg on 1st day of travel as single dose, followed by 100 mg daily. Prevention of leptospirosis 200 mg wkly. Treatment of leptospirosis 100 mg bd for 7 days.
Administration
May be taken with or without food: Take w/ a full glass of water & remain upright for at least 30 min. If gastric irritation occurs, take w/ food or milk.
Contraindications
Hypersensitivity to doxycycline or any tetracyclines.
Special Precautions
Discontinue use if severe skin reactions occur & at 1st evidence of skin erythema. Benign intracranial HTN (pseudotumor cerebri); pseudomembranous colitis. Clostridium difficile-associated diarrhea; overgrowth of nonsusceptible organisms; risk of esophageal irritation & ulceration. Increased vag candidiasis incidence. Ensure adequate hydration. Avoid exposure to direct sunlight or UV light. Monitor hematopoietic, renal & hepatic functions in long-term therapy. Perform mthly serological tests for at least 4 mth in venereal disease w/ co-existent syphilis. Avoid concomitant use w/ isotretinoin. Concomitant use w/ bismuth subsalicylate. Renal & hepatic impairment. Not to be used during pregnancy. Avoid use during lactation. Permanent teeth discoloration in childn.
Adverse Reactions
Hypersensitivity eg, anaphylactic shock & reaction, anaphylactoid reaction, angioedema, SLE exacerbation, pericarditis, serum sickness, Henoch-Schonlein purpura, hypotension, dyspnoea, tachycardia, peripheral oedema & urticaria; headache; nausea/vomiting; photosensitivity reaction, rash eg, maculopapular & erythematous rashes.
Drug Interactions
Prolonged prothrombin time w/ warfarin & other anticoagulants. Interfered bactericidal action of penicillin. Impaired absorption by Al, Ca & Mg-containing antacids, Fe-containing prep & bismuth salts. Decreased t½ w/ alcohol, barbiturates, carbamazepine & phenytoin. Fatal renal toxicity w/ methoxyflurane. Less effective OCs. Falsely elevated urinary catecholamine levels w/ fluorescence test.
