Propofol

Short-acting IV anaesth suitable for general anaesth induction & maintenance. Sedation of ventilated patients receiving intensive care; surgical & diagnostic procedures.

General anaesth induction Adult <55 yr 1.5-2.5 mg/kg. Childn >8 yr Approx 2.5 mg/kg. General anaesth maintenance Adult 4-12 mg/kg/hr by continuous infusion or 25-50 mg increments by repeat bolus inj. Childn 9-15 mg/kg/hr. ICU sedation Adult 0.3-4 mg/kg/hr by continuous infusion. Sedation for surgical & diagnostic procedures Adult 0.5-1 mg/kg over 1-5 min. Maintenance: 1.5-4.5 mg/kg/hr. Rapid increased depth of sedation 10-20 mg bolus inj.

Hypersensitivity to propofol, peanut or soya. ICU sedation of patients ≤16 yr.

Not to be administered via microbiological filter. Acute pulmonary insufficiency or resp depression; hypotension, transient apnoea; post-op unconsciousness; epileptic patients; cardiac & resp impairment, hypovolaemic patients; fat metabolism disorders. Increased risk of relative vagal overactivity. May cause involuntary movements. Patients undergoing procedures where spontaneous movements are undesirable eg, ophth surgery. High-dose & long-term use. Monitor for early signs of hypotension, airway obstruction & O₂ desaturation. Monitor blood lipid levels. Avoid alcohol prior to & for at least 8 hr after administration. Not recommended w/ electroconvulsive treatment. Concomitant use w/ CNS depressants eg, alcohol, general anaesth, narcotic & potent analgesics; other agents causing bradycardia. May affect ability to drive & use machines. Renal or hepatic impairment. Not be used for obstet anaesth & during pregnancy. Lactation. Not indicated for ICU sedation in patients ≤16 yr. Not recommended for paed general anaesth, in childn <1 mth & neonates. Not to be used as rapid bolus inj in elderly. Debilitated patients.

Local pain. Hypotension; bradycardia; transient apnoea during induction; nausea, vomiting; headache; w/drawal symptoms; flushing.

Additive sedative, anaesth & cardioresp depressant effects w/ other CNS depressants eg, premed drugs, inhalation & analgesic agents. Not to be given via same IV line w/ neuromuscular blockers eg, atracurium, mivacurium.
