Tramadol HCl
Indications/Uses
Dosage/Direction for Use
Adult & adolescent ≥12 yr 50-100 mg. Max: Not to exceed 400 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Acute intoxication w/ alcohol, hypnotics, centrally-acting analgesics, opioids, psychotropic drugs. MAOI or w/in 2 wk of w/drawal. Childn <18 yr w/ post-op pain after tonsils &/or adenoids removal. Adolescents 12-18 yr w/ obesity, obstructive sleep apnea or severe lung disease.
Special Precautions
Not suitable as substitute in opioid-dependent patients. Discontinue use if serotonin syndrome is suspected. Patients at risk for resp depression eg, substantially decreased resp reserve, hypoxia, hypercapnia, preexisting rep depression; head injury, increased ICP, patients prone to convulsive disorders or shock; history of epilepsy, patients susceptible to seizures; adrenal insufficiency. CYP2D6 ultra-rapid metabolisers. Long-term use. Concomitant use w/ drugs lowering seizure threshold; alcohol, other CNS depressants; benzodiazepines; serotonergic drugs. Severe hepatic & renal impairment; dialysis. Not recommended during pregnancy. Not to be used during lactation. Not recommended in childn <12 yr. Elderly >75 yr.
Adverse Reactions
Nausea, sweating, dry mouth, dizziness, giddiness, skin diaphoresis.
Drug Interactions
MAOIs or w/in 2 wk of w/drawal. Potentiated CNS depressant effects w/ other centrally-acting drugs. Increased potential for convulsions w/ SSRIs, TCAs. Serotonin syndrome w/ serotonergic drugs. Increased INR w/ coumarin anticoagulants. Decreased serum conc w/ carbamazepine. Increased risk of resp depression, profound sedation, coma & death w/ benzodiazepines.
