Indications/Uses
Dosage/Direction for Use
Cap Adult & adolescent >14 yr 50-100 mg 6 hrly. Max: 400 mg. PR cap Adult, elderly <75 yr & childn >12 yr 100-200 mg daily, may be increased until pain relief is attained. Inj Adult & childn >14 yr 100-400 mg daily by slow IV inj or infusion, or 100 mg IM or SC. Total daily dose: 400 mg. Childn >1 yr 1-2 mg/kg.
Administration
May be taken with or without food: Swallow whole, do not chew.
Contraindications
Hypersensitivity. Acute intoxication w/ alcohol, hypnotics, analgesics, opioids & psychotropics. Concomitant MAOIs or w/in 14 days or 2 wk of treatment. Epilepsy. Narcotic w/drawal treatment; substitute in opioid dependency. Severe hepatic insufficiency. Long-term use during lactation. Childn <1 yr.
Special Precautions
Not as substitute in opioid dependence. Patients w/ cranial trauma, prone to convulsive & biliary tract disorders, state of shock, altered state of consciousness, problem affecting resp center or function, increased ICP, existing resp depression or excessive bronchial secretion. Long-term use. Not for use in minor pain. Concomitant use w/ SSRIs, TCAs, antipsychotics, centrally-acting analgesics, local anaesth, opioid agonists-antagonists (nalbuphine, buprenorphine, pentazocine), CNS depressants. Not recommended in moderate to severe renal & moderate hepatic impairment. May affect ability to drive & use machines. Avoid use in pregnancy & lactation. Not recommended in childn <12 yr. Elderly >75 yr.
Adverse Reactions
Nausea; dizziness. Emesis, constipation, dry mouth; headache, incoherence; sweating.
Drug Interactions
Risk of serotoninergic syndrome w/ MAOIs, SSRIs, triptans. Increased sedative effect by alcohol. Decreased plasma conc by carbamazepine & other enzyme inducers. Decreased analgesic effect w/ opioid agonists-antagonists (buprenorphine, nalbuphine, pentazocine). Increased risk of resp & central depression w/ other opioid derivatives (ie, antitussive drugs & substitutive treatments), benzodiazepines & barbiturates, other anxiolytics, hypnotics, sedative antidepressants & antihistamines, neuroleptics, centrally-acting antihypertensives, thalidomide & baclofen. Increased INR w/ warfarin-like compd. Inhibited metabolism by CYP3A4 inhibitors eg, ketoconazole & erythromycin. Increased risk of convulsion w/ bupropion, serotonin re-uptake inhibitor antidepressants, TCAs & neuroleptics.
