Indications and Dosage
Vaginal
Vulvovaginal candidiasis
Adult: As 0.4 or 0.8% cream: Apply intravaginally at bedtime for 7 consecutive days or for 3 consecutive days, respectively. As 80 mg supp: Admin intravaginally at bedtime for 3 consecutive days.
Contraindications
Hypersensitivity.
Special Precautions
Discontinue if irritation, sensitisation, fever, chills or flu-like symptoms develop. Pregnancy and lactation.
Adverse Reactions
Anaphylaxis, toxic epidermal necrolysis. Vulvovaginal burning, pruritus, irritation, itching; headache, dysmenorrhoea; genital, body and abdominal pain; flu-like syndrome (e.g. fever, chills, hypotension), vertigo, nausea.
Patient Counseling Information
May damage rubber or latex condoms or diaphragms, avoid use while on treatment.
Action
Description:
Mechanism of Action: Terconazole disrupts ergosterol synthesis by binding to fungal cytochrome P450. It is active in vitro against Candida spp. and other fungi. It has some antibacterial activity in vitro but not against usual vag flora e.g. lactobacilli.
Pharmacokinetics:
Absorption: Absorbed systemically (5-16%).
Metabolism: Metabolised hepatically.
Excretion: Via urine and faeces.
