Indications/Uses
Essential HTN in adults. Reduce risk of MI, stroke or death from CV causes in patients ≥55 yr at high risk of developing major CV events who are unable to take ACE inhibitors.
Dosage/Direction for Use
Essential HTN 40 mg once daily, may be increased to max: 80 mg once daily. Severe HTN Up to 160 mg alone or in combination w/ hydrochlorothiazide 12.5-25 mg dally. CV prevention 80 mg once daily. Mild to moderate hepatic impairment Not to exceed 40 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Biliary obstructive disorders. Concomitant use w/ aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2). Severe hepatic impairment. 2nd & 3rd trimesters of pregnancy.
Special Precautions
Not recommended in dual blockade of renin-angiotensin-aldosterone system, primary aldosteronism. Bilateral renal artery stenosis or artery stenosis to single functioning kidney; intravascular hypovolaemia; severe CHF; aortic or mitral stenosis, obstructive hypertrophic cardiomyopathy; hyperkalemia; ischaemic cardiomyopathy or CV disease. Patients w/ intercurrent events eg, dehydration, acute cardiac decompensation, metabolic acidosis, infectious diseases, cellular lysis. Diabetic patients treated w/ insulin or antidiabetics. Black patients. Monitor K & serum creatinine levels periodically in patients w/ renal impairment. Not to be co-administered w/ aliskiren in diabetic patients. Not to be used concomitantly w/ ACE inhibitors in patients w/ diabetic nephropathy. Avoid use w/ aliskiren in patients w/ renal impairment (GFR <60 mL/min). Not recommended to be used w/ ACE inhibitor. Concomitant use w/ K-sparing diuretics, K supplements, K-containing salt substitutes, NSAIDs including COX-2 inhibitors, immunosuppressives, trimethoprim, other drugs increasing K level eg, heparin. Not to be given in patients w/ cholestasis, biliary obstructive disorders or severe hepatic impairment. Recent kidney transplantation; underlying renal disease, renal artery stenosis. Mild to moderate hepatic impairment. Not recommended during 1st trimester of pregnancy & lactation. Childn <18 yr. Elderly >70 yr.
Adverse Reactions
Hypotension.
Drug Interactions
Increased digoxin peak plasma & trough conc. Increased serum K w/ K-containing salt substitutes, K-sparing diuretics, K supplements, ACE inhibitors, heparin, immunosuppressives (cyclosporin, tacrolimus), trimethoprim. Reversible increased serum lithium conc & toxicity. Reduced antihypertensive effects w/ NSAIDs including COX-2 inhibitors; systemic corticosteroids. Vol depletion & increased risk of hypotension w/ high-dose diuretics. Not recommended in concomitant use w/ ramipril. Increased BP-lowering effect w/ other antihypertensives. Potentiated hypotensive effects w/ baclofen, amifostine. Aggravated orthostatic hypotension w/ alcohol, barbiturates, narcotics, antidepressants.
