Piperacillin 4 g, tazobactam 500 mg
Indications/Uses
Lower resp tract, complicated & uncomplicated UTI, intra-abdominal, polymicrobic, skin & skin structure infections; bacterial septicaemia. In combination w/ aminoglycoside for bacterial infections in neutropenic adults or childn.
Dosage/Direction for Use
Adult & childn ≥12 yr, <50 kg on haemodialysis 4.5 g every 8 hr. Total daily dose: 2.5-4.5 g every 6-8 hr. Neutropenia Adult & childn ≥12 yr, >50 kg 4.5 g every 6 hr in combination w/ aminoglycoside, <50 kg 90 mg/kg every 6 hr in combination w/ aminoglycoside. Intra-abdominal infection Childn 2-12 yr, up to 40 kg 112.5 mg/kg every 8 hr, >40 kg Adult dose. Adult & childn >50 kg CrCl 20-80 mL/min Piperacillin 12 g/tazobactam 1.5 g daily every 8 hr in divided doses, <20 mL/min Piperacillin 8 g/tazobactam 1 g every 12 hr in divided doses. Childn <50 kg CrCl 40-80 mL/min Piperacillin 80 mg/tazobactam 10 mg every 6 hr, 20-40 mL/min Piperacillin 80 mg/tazobactam 10 mg every 8 hr, <20 mL/min Piperacillin 80 mg/tazobactam 10 mg every 12 hr.
Contraindications
Hypersensitivity to penicillins, cephalosporins or β-lactamase inhibitors.
Special Precautions
History of penicillin, cephalosporin & other allergen hypersensitivity. Discontinue use if allergic reaction or bleeding manifestation occurs. Antibiotic-associated pseudomembranous colitis; superinfections. Neuromuscular excitability or convulsions (high-dose). Perform periodic assessment of renal, hepatic & haematopoietic function during prolonged therapy; electrolyte determination in patients w/ low K reserves or are receiving cytotoxic therapy or diuretics. Patients on restricted salt intake. Renal or hepatic impairment. Pregnancy & lactation.
Adverse Reactions
Diarrhea, nausea, vomiting; rash.
Drug Interactions
Longer t½ & lowered renal clearance w/ probenecid. Physical incompatibility w/ aminoglycoside. Frequently monitor & test coagulation parameters w/ high-dose heparin, oral anticoagulants. Prolonged neuromuscular blockade of vecuronium & similar neuromuscular-blocking agents. Reduced MTX clearance. False +ve reaction for glucose urine test. Chemical instability w/ Na bicarbonate.
