Per 2.25 g powd for inj Piperacillin Na 2 g, tazobactam Na 250 mg. Per 4.5 g powd for inj Piperacillin Na 4 g, tazobactam Na 500 mg
Indications/Uses
Systemic &/or local bacterial lower resp tract infection, complicated & uncomplicated UTI, intra-abdominal infections, skin & skin structure infections & bacterial septicaemia. In combination w/ aminoglycosides for bacterial neutropenic infection in adults & childn.
Dosage/Direction for Use
Adult & childn >12 yr 4.5 g 8 hrly. Total daily dosage: 2.25-4.5 g 6-8 hrly depending on severity of infection. Neutropenia Adult & childn <12 yr Recommended dose: 4.5 g 6 hrly in combination w/ aminoglycoside. Childn >50 kg Adult dose, <50 kg Piperacillin 80 mg/tazobactam 10 mg/kg 6 hrly in combination w/ aminoglycoside. Adult & childn >50 kg CrCl 20-80 mL/min Piperacillin 12 g/tazobactam1.5 g daily in divided doses of 4.5 g 8 hrly, <20 mL/min Piperacillin 8 g/tazobactam 1 g daily in divided doses of 4.5 g 12 hrly, <50 kg CrCl 40-80 mL/min Piperacillin 80 mg/tazobactam 10 mg/kg 6 hrly, 20-40 mL/min Piperacillin 80 mg/tazobactam 10 mg/kg 8 hrly, <20 mL/min Piperacillin 80 mg/tazobactam 10 mg/kg 12 hrly. Haemodialysis Adult Max: Piperacillin 8 g/tazobactam 1 g, additional 2 g/250 mg after each dialysis. Childn <50 kg Recommended dose: 45 mg/kg 8 hrly for 5-14 days.
Contraindications
Hypersensitivity to penicillins &/or cephalosporins or β-lactamase inhibitors.
Special Precautions
Discontinue use if serious hypersensitivity reactions, antibiotic-induced pseudomembranous colitis or bleeding manifestations occur. Perform periodic assessment of renal, hepatic & hematopoietic function. Perform periodic electrolyte determinations in patients w/ low K reserves. Evaluate patients for syphilis prior to treatment. Risk of superinfections; neuromuscular excitability or convulsions (high doses). Pregnancy & lactation.
Adverse Reactions
Phlebitis, thrombophlebitis. Diarrhea, rash, erythema, pruritus, vomiting, allergic reactions, nausea, urticaria, superinfections, skin reactions, eruption, increased sweating, erythema, multiforme, eczema, exanthema, maculopapular rash, soft/loose stools, stomatitis, constipation, muscular weakness, hallucination, dry mouth, hypotension, muscle pain, superficial phlebitis, fever, hot flushes, oedema, tiredness, local inj site inflammation & pain, leucopenia, eosinophilia, thrombocytosis, thrombocytopenia, +ve Coombs' test, hypokalemia, increased serum liver enzymes, bilirubin & renal function parameters.
Drug Interactions
Longer t½ & lowered renal clearance w/ probenecid. Monitor & frequently test coagulation parameters w/ high-dose heparin & oral anticoagulants. Prolonged action of vecuronium & other similar neuromuscular-blocking agents. Reduced excretion of MTX. Not to be used w/ Na bicarbonate-containing soln. Not to be added to blood products or albumin hydrolysates. Not to be mixed in IV soln w/ aminoglycosides.
